摘要 目的通过分析中性粒细胞与淋巴细胞的比值(NLR)在糖尿病肾病(DKD)中的变化,探讨NLR与DKD之间的关系。方法 120例2型糖尿病患者根据尿微量白蛋白/肌酐比值(UMA/Cr),分为正常白蛋白尿组(NA组)38例(UMA/Cr<30μg/mg);微量白蛋白尿组(MIA组)52例(UMA/Cr 31-300μg/mg);大量白蛋白尿组(MA)30例(UMA/Cr>300μg/mg);均合并糖尿病视网膜病变(DR)。同时随机选择本院健康体检者30例作为对照组,比较四组间白细胞总数、中性粒细胞、淋巴细胞、NLR、Cr、UA、TC、TG、HDL-C、LDL-C和Hb A1C。结果与对照组比较,NA组、MIA组、MA组NLR水平明显升高(P<0.05),且三组间比较差异也均有统计学意义(P<0.05)。TG、LDL-C、HBA1C、NLR、TG是DKD的危险因素(P<0.05)。结论NLR的升高与DKD有关,且NLR有可能成为预测早期DKD的新指标。 Objective To investigate the change of neutrophil / lymphocyte ratio( NLR) in the patients with diabetic kidney disease( DKD),and to explore the relationship between NLR and DKD. Methods A total of 120 diabetic kidney disease patients with diabetic retinopathy( DR) were recruited in this study,and they were divided into three groups according to the UMA / Cr ratio: normal albuminuria group( n = 38,NA group,UMA / Cr 3〈0 μg / mg),microalbuminuria group( n = 52,MIA group,UMA / Cr 31- 300 μg / mg),and macroalbuminuria group( n = 30,MA group,UMA / Cr 〉300 μg / mg). Thirty healthy subjects were randomly chosen as control group. The numbers of leukocyte,neutrophil,lymphocyte,as well as NLR,Cr,UA,TC,TG,HDL-C,LDL-C and Hb A1 C levels were compared among these four groups. Results Compared to control group,NLR level was significantly increased in NA group,MIA group and MA group( P〈0. 05),and there was significant difference between the three groups( P〈0. 05). TG,LDL-C,Hb A1 C,NLR,TG were risk factors of DKD( P〈0. 05). Conclusion The increase of NLR is related with DKD,and NLR may be a new indicator to predict early DKD.
