摘要
目的探讨中性粒细胞与淋巴细胞比值(NLR)在多发伤患者中的应用价值,辅助临床评估多发伤患者创伤严重程度以及对预后进行判断分析。方法将2016年1月至2018年12月龙华区中心医院急诊科收治的164例多发伤患者依据损伤严重程度评分(NISS)分为轻伤组(n=92)、重伤组(n=72),对比分析两组患者入院后前7d的NLR水平差异,并采用Spearman相关分析探讨NLR水平与NISS评分相关性。根据患者预后分为生存组(n=116)与死亡组(n=48),比较两组患者前7d的NLR水平差异,并利用受试者工作特征曲线分析NLR在多发伤患者预后评估中的价值。结果①轻伤组与重伤组患者比较,随着NISS评分升高,NLR呈现增高趋势,两组间差异显著(P<0.05);②死亡组患者NLR水平显著高于生存组,两组比较,差异显著(P<0.05),ROC曲线显示NLR预测多发伤患者预后具有较高的敏感度和特异度。结论NLR表达水平能较好的判断多发伤患者的伤情严重程度,并能较好的判断预后,可以作为良好的指标用于指导临床对于多发伤患者的诊治。
Objective To explore the value of neutrophil to lymphocyte ratio(NLR)in patients with multiple trauma,and to assist clinical assessment of the severity of trauma and prognosis of patients with multiple trauma.Methods 164 patients with multiple injuries were divided into mild injury group(n=92)and severe injury group(n=72)according to the NISS score from January 2016 to December 2018.The difference of NLR level between the two groups was compared and analyzed 7 days before admission,and the correlation between NLR level and NISS score was explored by Spearman correlation analysis.The patients were divided into survival group(n=116)and death group(n=48)according to their prognosis.The difference of NLR level in the first 7 days between the two groups was compared,and the value of NLR in prognosis evaluation of multiple trauma patients was analyzed by using the working characteristic curve of subjects.Result Compared with the severe injury group,NLR increased with the increase of NISS score,and the difference between the two groups was significant(P<0.05).The level of NLR in the death group was significantly higher than that in the survival group,and the difference between the two groups was significant(P<0.05).ROC curve showed that NLR had higher sensitivity and specificity in predicting the prognosis of multiple injury patients.Conclusion The expression of NLR can better judge the severity and prognosis of multiple trauma patients,and can be used as a good indicator to guide clinical diagnosis and treatment of multiple trauma patients.
