头孢克洛与头孢拉定治疗呼吸道感染的耐药趋势分析 被引量:11

摘要 目的分析头孢克洛和头孢拉定对呼吸道感染病原菌耐药性变化趋势,为临床抗菌药物应用提供参考依据。方法回顾性分析2012年和2013年2 907例在医院住院应用头孢克洛及头孢拉丁治疗的呼吸道感染患者为研究对象,比较两年病原菌检出率,对比头孢克洛和头孢拉定两年病原菌敏感性及清除率变化情况,数据采用SPSS 17.0软件进行统计分析。结果 2013年病原菌分布中假单胞菌属、假丝酵母菌属分别占23.59%、8.77%,高于2012年的13.68%、3.21%,检出率明显上升,2012年及2013年头孢拉定敏感率分别为86.0%、80.1%,低于同年头孢克洛敏感率的95.0%、93.0%(P<0.05);2012年及2013年头孢拉定清除率分别为73.9%、64.9%,低于同年头孢克洛清除率的82.2%、80.1%,头孢拉定2013年清除率低于2012年,差异有统计学意义(P<0.05)。结论 2012年和2013年呼吸道感染患者应用头孢克洛药物敏感性及病原菌清除率优于头孢拉定,头孢拉定病原菌清除率有所下降,根据患者病原菌药敏结果及抗菌药物应用史合理选择抗菌药物,是提高病原菌清除率的有效方法。 OBJECTIVE To analyze the changing trend in resistance of cefaclor and cefradine to pathogens of respir- atory tract infections so as to provide evidence for usage of clinical antimicrobial agents. METHODS A retrospec- tive analysis was conducted on 2 907 inpatients with respiratory infections treated cefaclor and cefradine in the year 2012 and 2013. The pathogen detection rates in the two years were compared. The pathogen sensitivity and the clearance changes in the two years were compared between cefaclor and cefradine. The SPSS17.0 statistical soft- ware was used for data analysis. RESULTS According to pathogen distribution, Pseudomonas and Candida ac- counted for 23.59% and 8.77% respectively in the year 2013, higher than 13.68% and 3.21% in 2012, showing a significantly increasing trend. The sensitive rate to cefradine was 86.0% and 80.1% in 2012 and 2013, lower than that of 95.0%. and 93.0% to cefaclor in the same years (P〈0.05). The clearance rate for cefradine was 73.9% and 64.9% respectively in 2012 and 2013, lower that of 82.2% and 80.1% for cefaclor in the same years. The clearance rate in 2013 was lower than thet in 2012 for cefradine (P〈0.05). CONCLUSION For patients with respiratory infections in 2012 and 2013, the susceptibility and the pathogen clearance was better for cefaclor than thet for cefradine. The pathogen clearance rate for cefradine was dropped. A reasonable choice of antibiotics should be made based on pathogen susceptibility results of the patients and the history of antimicrobial agent us- age, which is an effective way to improve the pathogen clearance.
