早期非小细胞肺癌伽玛刀与三维适形及调强放疗的应用探讨 被引量:4

摘要 目的对早期非小细胞肺癌患者应用三种不同的放疗方法,比较剂量学特点、以及危及器官的受量,评估其优缺点。方法选择20例早期非小细胞肺癌患者,同时应用伽玛刀(γ刀)、三维适形放疗(3D-CRT)、调强放射治疗(IMRT)制定计划,计算计划靶区(PTV)的适形指数和均匀指数及危及器官受照面积。结果3D-cRT与IMRT的PTV在适形度和均匀性上优于γ刀,三套计划在食管及脊髓Dmax方面无差异,在心脏受量方面,IMRT降低了v40的有效体积。γ刀显著降低MLD、V20、V30的有效体积,但其肺部的V5大于3D-CRT和IMRT。结论γ刀治疗早期非小细胞肺癌可获得较好的疗效且不良反应轻微,但计划的均匀性和适形度差,且对于放疗精度、靶区勾画要求较高。
