研究肺心病急性期患者凝血、抗凝及纤溶系统功能的变化 被引量:6

摘要 目的研究肺心病急性期患者血液流变学、凝血、抗凝及纤溶系统功能的变化。方法于2015年1月至2016年10月收集慢性肺心病60例,其中肺心病缓解期30例,肺心病急性期30例,另外选取30例健康人群作为对照组,比较肺心病组和对照组的凝血、抗凝、纤溶指标、免疫指标。结果①和对照组对比,肺心病缓解期患者APTT更低,D-D、FVⅢ更高,差异存在统计学的意义,P <0.05;PT差异无统计学意义,P>0.05。肺心病急性期患者PT、APTT更低,FVⅢ、D-D更高,差异存在统计学的意义,P<0.05。和缓解期对比,急性期患者PT、APTT更低,D-D、FVⅢ更高,差异存在统计学的意义,P<0.05。②对照组、肺心病缓解期两组C3、C4、IgA、IgG、IgM差异无统计学意义,P>0.05。和对照组比较,肺心病急性期C3、C4、IgA、IgG、IgM更低,P<0.05。结论凝血指标、免疫炎症因子和肺心病病情的关系密切,对肺心病患者进行C3、C4、IgA、IgG、IgM、D-D水平、凝血指标的检测,有利于尽早发现和监测患者的病情发展情况。 Objective To investigate the changes of coagulation, anticoagulation and fibrinolysis system in patients with cor pulmonale at acute stage. Methods From January 2015 to October 2016,60 cases with chronic pulmonary heart disease were collected,including 30 cases of pulmonary heart disease in the remission stage and 30 cases of pulmonary heart disease in acute phase. 30 healthy cases were selected as control group.Blood coagulation,anticoagulation,fibrinolysis index and immune index were compared between two groups.Results ①Compared with the control group,the APTT in patients with cor pulmonale at remission stage was significantly lower,the D-D and FVⅢ were significantly higher,the data difference was obvious,and there was no significant difference(P >0. 05). In the acute phase of cor pulmonale patients,PT and APTT levels were significantly lower, FVⅢ and D-D levels were significantly higher, and the data were significantly different, P < 0. 05. Compared with remission stage, PT and APTT in patients with acute stage were significantly lower,D-D and FVⅢ were significantly higher,and the difference of data was obvious,P <0.05.②There was no significant difference in the levels of C3,C4,IgA,IgG and IgM between the two groups in the control group and the remission stage of pulmonary heart disease,P >0.05. Compared with the control group,levels of C3,C4,IgA,IgG and IgM in the acute phase of cor pulmonale were significantly lower,P <0.05. Conclusion The relationship between coagulation index, immune inflammatory factors and pulmonary heart disease disease closely,detective C3,C4,IgA,IgG,IgM,D-D level,blood coagulation index of patients with pulmonary heart disease,are conducive to the discovery and monitoring the progress of disease as soon as possible.
