低分子肝素对慢性阻塞性肺疾病合并肺心病急性期患者血清脑钠肽、D-二聚体水平的影响 被引量:13

摘要 目的探讨低分子肝素治疗慢性阻塞性肺疾病(COPD)合并肺心病急性期的疗效及血清脑钠肽(BNP)、D-二聚体水平变化。方法选取COPD合并肺心病急性期患者60例,随机分为研究组和对照组,各30例。对照组患者给予常规治疗,研究组患者在对照组治疗基础上给予低分子肝素治疗,观察并比较2组患者治疗效果及血清BNP、D-二聚体水平变化。结果研究组患者总效率明显高于对照组患者(χ~2=7.9542,P<0.05);治疗后,研究组患者p(O_2)和p(CO_2)均优于对照组(t_(p(O2))=5.9399、t_(p(CO_2))=-6.0714,P<0.05);治疗后,研究组患者D-二聚体水平低于对照组(t=-3.3850,P<0.05);研究组患者治疗后BNP水平下降更显著(t=-8.6173,P<0.05)。结论COPD合并肺心病急性期患者在常规治疗基础上配合低分子肝素治疗能够取得满意的临床效果,明显改善患者血气指标和肺功能。 Objective To investigate the efficacy of low molecular weight heparin in the treatment of chronic obstructive pulmonary disease(COPD) with pulmonary heart disease and the changes of serum brain natriuretic peptide (BNP) and D-dimer levels. Methods A total of 60 patients with chronic obstructive pulmonary disease complicated with pulmonary heart disease were selected, and were randomly divided into study group and control group, with 30 cases per group. The patients in the control group were given routine treatment, and those in the study group were treated with low molecular weight heparin on the basis of the control group. The therapeutic effect and serum BNP and D-dimer levels of the two groups were observed and compared. Results The total effective rate of the study group was significantly higher than that of the control group ( χ 2=7.9542, P 〈0.05). After the treatment, p (O 2) and p (CO 2) levels in the study group were significantly better than that in the control group ( t p (O 2) = 5.9399 , t p (CO 2) =-6.0714, P 〈0.05). After the treatment, the level of D-dimer in the study group was lower than that in the control group ( t =-3.3850, P 〈0.05), and the level of BNP decreased significantly in the study group ( t =-8.6173, P 〈0.05). Conclusion The patients with COPD complicated with pulmonary heart disease on the basis of conventional treatment with low molecular weight heparin treatment can achieve satisfactory clinical results, and significantly improve the patient′s blood gas and lung function.
