微粒子化学发光法检测抗环瓜氨酸肽抗体诊断试剂的诊断效能评价 被引量：3

摘要 目的评价1种新的微粒子化学发光法(CMIA)抗环瓜氨酸肽抗体(抗CCP)试剂的诊断性能。方法用雅培AR-CHITECT CMIA法抗CCP抗体试剂检测554例样本的抗CCP抗体,评价其诊断性能。结果 450例类风湿性关节炎(RA)患者中397例抗CCP抗体阳性,诊断灵敏度为88.2%。104例对照样本中12例抗CCP抗体阳性,诊断特异度为88.5%。类风湿因子(RF)和抗角蛋白抗体(AKA)对RA的诊断灵敏度分别为80.2%(361/450)和60.3%(210/348),特异度分别为56.7%(59/104)和96.9%(94/97)。抗CCP抗体含量大于19.3U/mL时,诊断RA的灵敏度和特异度分别为77.1%和95.3%。结论 CMIA法抗CCP抗体对RA的诊断性能优于RF和AKA。抗CCP抗体含量较高时,对RA的诊断特异度更高。
