普萘洛尔对婴幼儿血管瘤组织中血管内皮生长因子表达的影响 被引量:14

Influence of Oral Propranolol on Expression of VEGF in Hemangioma Tissues of Infants

摘要 目的探讨普萘洛尔对婴幼儿血管瘤组织血管内皮生长因子(VEGF)表达的影响及临床意义。方法选取拟手术治疗的增生期血管瘤患儿20例,随机分为两组,分别于术前4周(10例)、3个月(10例)口服普萘洛尔1.0 mg/(kg.d)各10例;对照组为消退期血管瘤10例和增生期血管瘤10例,均不予用药。比较4组手术切除后瘤体标本,VEGF表达的异常。结果增生期用药组VEGF表达呈阴性或阳性表达,而增生期未用药组VEGF呈强阳性表达。4组免疫反应阳性颗粒积分A值差异有统计学意义(F=17.7024,P<0.05),普萘洛尔影响时间和血管瘤VEGF的表达呈显著负相关(r=-0.5922,P<0.05)。结论应用普萘洛尔能明显下调增生期血管瘤组织VEGF的表达水平,可促进血管瘤的消退,因此有望用于治疗婴幼儿增生期血管瘤。 Objective To explore the effect of propranolol on the expression of vascular endothelial growth factor(VEGF) in the hemangioma tissues of infants and its clinical significance. Methods A total of 20 patients with proliferative hemangioma in the experiment group were divided into two subgroups: 10 patients in the experimental group A were injected with propranolol 4 weeks before operation, while 10 patients in the experimental group B were done 3 months before operation, the dose was 1.0 mg/ (kg · d) in both groups). 10 cases with involuting hemangioma( control group A) and 10 cases with proliferative hemangioma (control group B) were not treated with the propranolol. After the operations, the levels of VEGF and histologic specimens were compared among the four groups. Results VEGF in the experimental group A was negative or positive, but in the control group B was strongly positive expression. VEGF could be down-regnlated by the propranolol (F = 17. 7024, P 〈 0.05 ) ;the affection time and the expression of the VEGF by oral propranolol was obviously negative correlation( r = -0.5922, P 〈 0.05 ). Conclusion Propranolol can down-regulate the expression of VEGF of proliferative hemangioma tissues, relieve the hemangioma, and should be expected to use for the hemangioma in infants.
