摘要
目的探讨血浆上皮细胞钙黏蛋白(CDHl)基因及Ras相关区域家族1A基因(RASSF1A)甲基化对食管癌手术彻底程度的参考价值及其与食管癌临床病理特征的关系。方法应用Real-time甲基化特异性聚合酶联反应技术(MSP)技术对76例食管鳞癌患者肿瘤组织、配对的癌旁正常组织、术前、术中及术后外周血游离DNA中CDHl及RASSF1A基因的甲基化状态进行检测。结果CDH1及RASSF1A甲基化率在食管癌肿瘤组织中分别是72.37%、55.26%;在血浆中分别是63.16%、50.00%,且与病理分期、淋巴结转移及神经脉管浸润显著相关(P值均〈0.01);术前、术中、术后血浆中DNA甲基化发生率呈现先升后降的变化趋势。结论食管鳞癌患者外周血游离DNA中CDHl及RASSF1A甲基化可反映机体的荷瘤状态,并可作为食管鳞癌手术彻底程度的标志物。
Objective To investigate the relationships between E-eadherin (CDH1) and Ras association domain family 1A (RASSFI A) methylation in peripheral blood and clinicopathologic character of esophageal squamous cell carcinoma (ESCC). Methods Real-time methylation specific polymerease chain reaction (MSP) technique was used to detect methylation status of CDH1 and RASSF1A in tumour tissue, adjacent normal tissue and peripheral blood preoperatively in 76 cases of ESCC. Results CDH1 and RASSF1A methylation rate was 72. 37% and 55.26% in tumour tissue, and 63.16% and 50. 00% in pe- ripheral blood, respectively, and was significantly related to pathology stage, lymph nodes metastasis and nerve vessel invasion ( all P 〈 0. 01 ). DNA methylation rates in peripheral blood was increased initially and then decreased preoperatively, intraoperatively and postoperatively. Conclusion CDH1 and RASSF1A methylation in peripheral blood in patients with ESCC could reflect the tumor-bearing state of bodies. And CDH1 and RASSF1A mathylation may be a valuable diagnostic tool as a tumor marker for resection degree in patients with ESCC.
