小于胎龄儿出生后48—72小时血清脂肪酸成分的改变 被引量:2

CHANGES IN SERUM FATTY ACID COMPOSITION DURING 48-72 HOURS AFTER BIRTH IN SMALL INFANTS FOR GESTATIONAL AGE

摘要 本文用气相色谱法测定了29例小于胎龄儿(SGA)和57例适于胎龄儿(AGA)脐血清及出生后48~72小时的静脉血清总脂肪酸(TFA)和游离脂肪酸(FFA)的成分,以了解SGA儿出生后脂肪酸代谢特点,更好地指导喂养。 SGA儿和AGA儿生后48~72小时静脉血清C18∶2 φ6、C20∶4φ6、C18∶3φ和C22∶6φ3百分含量显著低于出生时水平,可能系营养源改变、体内有关去饱和酶活性不足及脑组织对必需脂肪酸(EFA)之需求所致,提示生后一段时间的SGA儿与AGA儿都有可能发生EFA缺乏。生后AGA儿血清EFA浓度显著高于脐血清,而SGA此时浓度与出生时无明显差异,提示SGA儿更易发生EFA缺乏,须强调出生后喂养。 Serum total fatty acid (TFA) and free fatty acid (FFA) composition of cords and veins were analysed in 29 small for gestational age (SGA) and 57 appropriate gestational age (AGA) infants at deliveries and 48-72 hours after birth, respectively in order to understand the feature of fatty acid metabolism in SGA infants and to improve the feeding measures after birth.The percentages of Cl8:2ω6, Cl8:3ω3, C20:4ω6 and C22:6ω3 in SGA and AGA infants 48-72 hours after birth are significantly lower than those at birth, probably as the results of change in nutritional sources, deficiencies of activities of concerned desaturases in neonates, and the needs of essential fatty acids (EFA) by brain tissue. So it is possible that SGA and AGA infants suffer from EFA deficiency during a short period after birth.The levels of serum EFAs in the AGA infants 48-72 hours after birth are significantly higher than those at birth, while the levels of EFAs in SGA infants have no differences at and after birth, suggesting that the SGA infants are more susceptible to EFA deficiency, so the rationalization of lipids feeding after birth is recommended.
