ZIC1在宫颈鳞状上皮癌患者中的表达水平及临床意义 被引量:1

Expression level and clinical significance of ZIC1 in patients with cervical squamous cell carcinoma

摘要 目的探讨ZIC1在宫颈鳞状上皮癌(CSCC)患者中的表达水平及临床意义,为临床诊治CSCC提供参考依据。方法经昆山市第一人民医院伦理委员会批准,采用RT-q PCR检测新鲜冷冻活检组织中ZIC1 mRNA的表达情况,免疫组化(SP)检测80例CSCC组织及相应的癌旁正常组织中ZIC1蛋白的表达情况。分析了ZIC1表达与CSCC临床病理特征的关系。采用Cox回归分析和Log秩检验的Kaplan-Meier曲线分析其预后价值。结果 CSCC中ZIC1 mRNA表达水平明显低于癌旁正常组织或CINⅠ~CINⅢ,差异有统计学意义(均P<0. 001)。CSCC与癌旁正常组织ZIC1半定量评价系统评分(IRS)呈负相关(r=-0. 279 3,P=0. 012),CSCC中ZIC1的平均评分(IRS=5. 36±3. 48)明显低于癌旁正常组织(IRS=11. 31±5. 68,P<0. 001)或CINⅢ组织(IRS=10. 42±1. 54,P<0. 001),差异均有统计学意义。此外,ZIC1阳性表达与FIGO分期(P=0. 027)和淋巴结转移呈负相关(P<0. 001)。Cox回归分析中,ZIC1表达(HR=0. 61,95%CI:0. 40~0. 92,P=0. 018)、FIGO分期(HR=3. 55,95%CI:2. 35~5. 37,P<0. 001)、淋巴结转移(HR=2. 50,95%CI:1. 62~3. 86,P<0. 001)是影响总体生存的3个独立预后因素。此外,ZIC1表达也与无病生存相关(P=0. 003)。结论 CSCC中ZIC1表达明显低于癌旁正常组织或CIN组织,ZIC1阳性表达与FIGO分期及淋巴结转移呈负相关。此外,ZIC1是一种有前途的CSCC预后生物标志物。 Objective To explore the expression level and clinical significance of ZIC1 in patients with cervical squamous cell carcinoma(CSCC),provide a reference basis for clinical diagnosis and treatment of CSCC.Methods With the approval of Ethics Committee of the First People’s Hospital of Kunshan City,RT-q PCR was used to detect the expression of ZIC1 mRNA in fresh-frozen biopsy tissues,immunohistochemical SP method was used to detect ZIC1 protein expression in 80 CSCC tissues and corresponding normal tissues adjacent to CSCC,the relationship between ZIC1 expression and clinicopathological characteristics of CSCC was analyzed.Cox regression analysis and Kaplan-Meier curve of Log rank test were used to analyze the prognostic value.Results The expression level of ZIC1 mRNA in CSCC was statistically significantly lower than those in normal cervical tissues or CINⅠ-CINⅢ(P<0.001).CSCC was negatively correlated with IRS of ZIC1 in corresponding normal tissues adjacent to CSCC(r=-0.279 3,P=0.012).The average score of ZIC1 in CSCC(IRS=5.36±3.48)was statistically significantly lower than that in corresponding normal tissues adjacent to CSCC(IRS=11.31±5.68,P<0.001)or CINⅢtissues(IRS=10.42±1.54,P<0.001).Positive expression of ZIC1 was negatively related to FIGO stage(P=0.027)and lymph node metastasis(P<0.001).Cox regression analysis showed that ZIC1 expression(HR=0.61,95%CI:0.40-0.92,P=0.018),FIGO staging(HR=3.55,95%CI:2.35-5.37,P<0.001),and lymph node metastasis(HR=2.50,95%CI:1.62-3.86,P<0.001)were independent prognostic factors of overall survival.ZIC1 expression was also correlated with disease-free survival(P=0.003).Conclusion ZIC1 expression in CSCC is significantly lower than that in normal cervical tissues or CIN tissues,positive expression of ZIC1 is negatively related to FIGO stage and lymph node metastasis.ZIC1 is a promising biomarker for prognosis of CSCC.
