摘要
目的:观察活血接骨汤联合植骨法治疗肱骨干骨折不愈合的临床效果。方法:选取2017年4月-2018年2月在笔者所在医院骨科治疗的74例肱骨干骨折不愈合患者作为本研究对象,按照随机数字表法分为对照组与治疗组各37例。对照组采用植骨法治疗,治疗组应用活血接骨汤联合植骨法治疗。比较两组患者的临床疗效、术前和术后1、3个月疼痛程度评分及术后并发症情况。结果:治疗组治疗优良率明显高于对照组(94.59%vs 75.67%),差异有统计学意义(χ~2=5.232,P=0.022)。两组患者术前疼痛评分差异无统计学意义(P>0.05),术后1、3个月,治疗组的疼痛评分明显低于对照组,差异有统计学意义(P<0.05)。治疗组术后并发症发生率(10.81%)明显低于对照组(29.73%),差异有统计学意义(χ~2=4.097,P=0.043)。结论:活血接骨汤联合植骨法治疗肱骨干骨折不愈合效果确切,降低术后并发症,增强成骨细胞和破骨细胞的活性,改善血液循环,加快骨痂形成进程,促进肌肉及关节恢复。
Objective:To investigate the effect of Huoxue Jiegu Decoction combined with bone grafting in the treatment of nonunion of humeral shaft fractures.Method:From April 2017 to February 2018,74 patients with nonunion of humeral shaft fractures treated in our hospital were selected as the subjects of this study,and randomly divided into the control group and the treatment group with 37 cases in each group.The control group was treated with bone grafting,and the treatment group was treated with Huoxue Jiegu Decoction combined with bone grafting.The clinical efficacy,preoperative and postoperative pain scores and postoperative complications were compared between the two groups.Result:The excellent and good rate in the treatment group was significantly higher than that in the control group(94.59% vs 75.67%, χ~2=5.232,P=0.022).There was no significant difference in preoperative pain score between the two groups(P>0.05).The pain score of the treatment group was significantly lower than that of the control group at 1 and 3 months after operation(P<0.05).The incidence of postoperative complications in the treatment group(10.81%) was significantly lower than that in the control group(29.73%),there was significant difference( χ~2=4.097,P=0.043).Conclusion:The Huoxue Jiegu Decoction combined with bone grafting has a definite therapeutic effect on nonunion of humeral shaft fractures,which can reduce postoperative complications,enhance the activity of osteoblasts and osteoclasts,improve blood circulation,accelerate callus formation process,and promote the recovery of muscles and joints.
