口腔种植修复技术对慢性牙周炎患者种植体松动度PD及SBI指数影响 被引量:19

摘要 目的探讨慢性牙周炎牙列缺损患者行口腔种植修复技术后的近、远期疗效。方法选取2016年3月-2017年12月本院口腔科门诊及住院收治的64例慢性牙周炎合并牙列缺损行种植修复术(共97枚种植体)患者为研究组,选择同期收治的64例牙周健康合并牙列缺损行种植修复术(共92枚种植体)患者为对照组,两组患者均于术后6个月、1年进行随访,比较其牙周探诊深度(PD)、龈沟出血指数(SBI)、菌斑指数(PLI)及龈沟液炎性因子(IL-6、IL-8)水平,同时于末次随访观察其种植体松动及脱落情况。结果术后6个月,研究组患者PD、SBI、PLI等指标与对照组比较(P> 0.05);术后1年,研究组患者PD、SBI、PLI等指标与对照组比较(P <0.05);研究组患者术后1年PD、SBI、PLI等指标与术后6个月比较(P <0.05);而对照组患者术后1年PD、SBI、PLI等指标与术后6个月比较(P> 0.05)。术后6个月,研究组患者IL-6、IL-8等龈沟液炎性因子水平与对照组比较(P <0.05);术后1年,研究组患者IL-6、IL-8等龈沟液炎性因子水平与对照组比较(P <0.05);研究组患者术后1年IL-6、IL-8等龈沟液炎性因子水平与术后6个月比较(P <0.05);而对照组术后1年IL-6、IL-8等龈沟液炎性因子水平与术后6个月比较(P> 0.05)。术后1年,研究组患者种植体留存率97.94%、脱落率2.06%与对照组95.65%、4.12%比较差异无统计学意义(P> 0.05);研究患者种植体松动度与对照组比较,其差异无统计学意义(P> 0.05)。结论慢性牙周炎牙列缺损患者行种植修复术后短期内种植体松动脱落情况与牙周健康者比较,其差异不显著,即短期疗效令人满意,但远期疗效需更多研究进一步证实。 Objective To investigate the short-term and long-term effects of oral implant repair in patients with chronic periodontitis.Methods Sixty-four patients with chronic periodontitis combined with dentition defect and implant surgery(a total of 97 implants) from March 2016 to December 2017 in our hospital were selected as the study group and selected for treatment at the same time. 64 patients with periodontal health and dentition defects were implanted with 92 implants as a control group. Both groups were followed up for 6 months and 1 year to compare the depth of their periodontal probing(PD). The levels of SBI, PLI and inflammatory factors(IL-6, IL-8)in gingival crevicular fluid were also observed. At the same time, the loosening and shedding of implants were observed at the last follow-up. Results At 6 months after operation, PD,SBI and PLI of the study group were compared with those of the control group(P>0.05). At 1 year after surgery, PD, SBI, and PLI of the study group were compared with those of the control group(P < 0.05).;PD, SBI,PLI and other indicators in the study group at 1 year after surgery compared with 6 months after operation(P < 0.05);and 1 year postoperative PD, SBI, PLI and other indicators in the control group and after 6 months Comparison(p>0.05). Six months after surgery,IL-6, IL-8, and other inflammatory factors in the gingival crevicular fluid of the study group were compared with those of the control group(P < 0.05);1 year after surgery, IL-6,IL-8,etc. were detected in the study group. The level of inflammatory factors in the gingival crevicular fluid was compared with that of the control group(P < 0.05);the level of inflammatory factors in the gingival crevicular fluid such as IL-6 and IL-8 in the study group was compared with that in the postoperative 6 months(P< 0.05). In the control group, IL-6,IL-8, and other inflammatory factors in the gingival crevicular fluid at 1 year after surgery were compared with 6 months after operation(P> 0.05).
