摘要
目的 初步探讨外周血干细胞移植预处理对骨髓造血微环境影响的分子机制。方法 采用流式细胞仪检测了 4 2例患者接受外周血干细胞移植预处理前及移植后 30、90、1 80d骨髓微环境基质细胞表达血管细胞粘附分子 (VCAM 1 )、纤维连接素 (Fn)、层粘素 (Ln)和Ⅳ型胶原 (ColⅣ )等细胞粘附分子水平的变化。结果 移植前接受单纯化疗方案预处理的患者 (简称单化组 )骨髓基质细胞表达VCAM 1、Fn、Lm和ColⅣ的水平以移植后 90d最低 ;移植前接受化疗 +全身放疗方案预处理的患者 (简称化放组 )骨髓基质细胞表达VCAM 1、Fn、Lm和ColⅣ的水平以移植后 30d最低 ;至移植后 1 80d ,单化组和化放组患者骨髓基质细胞表达VCAM 1、Fn、Lm和ColⅣ等指标仍未恢复到移植预处理前的水平 ,且放化组骨髓基质细胞表达上述细胞粘附分子水平的恢复过程较单化组缓慢。结论 造血干细胞移植前接受大剂量化 /放疗预处理后骨髓基质细胞表达细胞粘附分子的功能障碍可能与移植后某些患者造血功能恢复过程缓慢有关。
Objective To investigate the cell adhsion molecules(CAMs)expression on the stromal cells from bone marrow hematopoietic inductive microenvironment in 42 patients cross peripheral blood stem cell transplantation (BPSCT).Methods The CAMs including VCAM 1(vascular cell adhsion molecule 1),Fn(fibronectin),Ln(laminin)and Col Ⅳ(collagen Ⅳ)expression on bone marrow stromal cells cultured in vitro were detected by flow cytometry from patients with pretreatment scheme of high dose chemotherapy and high dose chemotherapy combined with total body irradiation therapy(TBI)respectively.Results The molecules mentioned above were lowest at 90 days post PBSCT from patients with simple chemotherapy. Otherwise, the same parameters showed lowest level at 30 days post PBSCT from patients with chemotherapy combined with TBI compared with that of themselves before pretreatment scheme( P <0.05).At the 180 days post PBSCT, these molecules didn't reach normal level in patients with pretreatment scheme of high dose chemotherapy and high dose chemotherapy combined with TBI respectively.Conclusion The dysfunction of adhsion molecules expression on bone marrow stromal cells may be one of the important factor for long term function obstacle of hematopoiesis in some patients with high dose chemotherapy or/and TBI pre BPSCT.
