摘要
目的分析血清纤维蛋白原(Fbg)、白三烯B4(LTB4)、C-反应蛋白(CRP)及降钙素原(PCT)检测在细菌性肺炎患者病情诊断及预后评估中的应用价值。方法选取2020年1月至2023年1月收治的80例细菌性肺炎患者作为观察组,选取50例检查健康者作为健康组。比较两组研究对象、观察组不同时间、观察组不同肺炎严重指数(PSI)分级的血清Fbg、LTB4、CRP及PCT水平。结果观察组的Fbg、LTB4、CRP及PCT水平均显著高于健康组,差异具有统计学意义(P<0.05)。出院时,观察组的Fbg、LTB4、CRP及PCT水平均显著低于治疗前、治疗3 d后、治疗7 d后,差异具有统计学意义(P<0.05)。根据PSI分级将观察组患者分为Ⅰ级28例、Ⅱ级18例、Ⅲ级15例、Ⅳ级12例、Ⅴ级7例。Ⅴ级患者的Fbg、LTB4、CRP及PCT水平均显著高于Ⅰ、Ⅱ、Ⅲ、Ⅳ级,差异具有统计学意义(P<0.05)。结论血清Fbg、LTB4、CRP及PCT检测用于细菌性肺炎患者的病情诊断与预后评估中,可取得较高的应用价值。
Objective To analyzed the application value of serum fibrinogen(Fbg),leukotriene B4(LTB4),C-reactive protein(CRP)and procalcitonin(PCT)detection in the disease diagnosis and prognosis evaluation of patients with bacterial pneumonia.Methods A total of 80 patients with bacterial pneumonia admitted from January 2020 to January 2023 were selected as observation group,and 50 healthy people were selected as healthy group.The levels of serum Fbg,LTB4,CRP and PCT in the two groups,the observation group at different times and the observation group with different Pneomonia Severity Index(PSI)grades were compared.Results The levels of Fbg,LTB4,CRP and PCT in the observation group were significantly higher than those in the healthy group,and the differences were statistically significant(P<0.05).At discharge,the levels of Fbg,LTB4,CRP and PCT in the observation group were significantly lower than those before treatment,after 3 d and 7 d of treatment,and the differences were statistically significant(P<0.05).According to the PSI classification,the patients in the observation group were divided into 28 cases of gradeⅠ,18 cases of gradeⅡ,15 cases of gradeⅢ,12 cases of gradeⅣand 7 cases of gradeⅤ.The levels of Fbg,LTB4,CRP and PCT in patients with gradeⅤwere significantly higher than those in gradeⅠ,Ⅱ,ⅢandⅣ,and the differences were statistically significant(P<0.05).Conclusion The detection of serum Fbg,LTB4,CRP and PCT can achieve high application value in the disease diagnosis and prognosis evaluation of patients with bacterial pneumonia.
