摘要
目的分析改良根治术与保乳术对早期乳腺癌患者美容效果与生存质量的影响。方法选取2020年1月至2021年12月萍乡市人民医院收治的80例早期乳腺癌患者作为研究对象,根据手术方式的不同分为改良根治术组(n=44)与保乳术组(n=36),比较两组临床指标、美容效果、生存情况与生存质量。结果保乳术组术中出血量均少于改良根治术组,手术时间与住院时间均短于改良根治术组,差异有统计学意义(P<0.05);保乳术组美容优良率为91.67%,高于改良根治术组的59.09%,差异有统计学意义(P<0.05);两组组无病生存率、局部复发率与远端转移率比较差异无统计学意义;保乳术组生理功能、生理职能、情感职能、心理健康、躯体疼痛、社会功能生命活力、总体健康评分及平均分均高于改良根治术组,差异有统计学意义(P<0.05)。结论改良根治术与保乳术治疗早期乳腺癌患者生存情况相似,但保乳术可减少术中出血量,缩短手术时间、住院时间,美容效果更佳,生存质量更高,可推荐作为早期乳腺癌的首选治疗方式。
Objective To analyze the effect of modified radical mastectomy and breast conserving surgery on the cosmetic effect and quality of life of early patients with breast cancer patients.Methods Eighty patients with early breast cancer who were admitted to Pingxiang People's Hospital from January 2020 to December 2021 were selected as the study subjects,and they were divided into modified radical mastectomy group(n=44)and breast conserving surgery group(n=36)according to the different surgical methods,the clinical indicators,cosmetic effects,survival conditions and quality of life were compared between the two groups.Results The intraoperative bleeding in the breast conserving surgery group was less than that in the modified radical surgery group,and the operation time and hospital stay were shorter than those in the modified radical surgery group,with statistical significance(P<0.05);The cosmetic excellence rate in the breast conserving surgery group was 91.67%,which higher than 59.09%in the modified radical surgery group,and the difference was statistically significant(P<0.05);There was no statistically significant difference in disease-free survival rate,local recurrence rate,and distant metastasis rate between the two groups;the physiological function,physiological function,emotional function,psychological health,physical pain,social function,vitality,overall health score,and average score in the breast conserving surgery group were higher than those in the modified radical surgery group,and the difference was statistically significant(P<0.05).Conclusion The survival of patients with early breast cancer treated with modified radical mastectomy and breast conserving surgery is similar,but breast conserving surgery can reduce the amount of intraoperative bleeding,shorten the operation time and hospital stay,and has better cosmetic effect and higher quality of life,which can be recommended as the first choice of treatment for early breast cancer.
