乳酸菌阴道胶囊联合克霉唑阴道片治疗妊娠期霉菌性阴道炎的临床效果

Clinical effect of lacidophilin vaginal capsules combined with clotrimazole vaginal tablets in the treatment of mycotic vaginitis of pregnancy

摘要目的观察乳酸菌阴道胶囊联合克霉唑阴道片治疗妊娠期霉菌性阴道炎的临床效果。方法回顾性选取2021年3月—2023年4月第九〇九医院(厦门大学附属东南医院)妇产科收治的妊娠期霉菌性阴道炎患者160例,根据治疗方法不同分为联合组(88例)和克霉唑组(72例)。克霉唑组采用克霉唑阴道片治疗,联合组在克霉唑组基础上联合乳酸菌阴道胶囊治疗。比较2组治疗总有效率、临床症状缓解时间,治疗前、停药1周后血清炎性因子[白介素-8(IL-8)、白介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]水平、阴道微生态指标[阴道pH值、白介素-1β(IL-1β)、乳酸脱氢酶(LDH)、分泌型免疫球蛋白A(sIgA)]变化及不良反应。结果联合组治疗总有效率为95.45%,高于克霉唑组的83.33%(χ^(2)=6.465,P=0.011);联合组白带异常、阴道瘙痒、阴道黏膜充血及阴道疼痛缓解时间均短于克霉唑组(P<0.01)。停药1周后,2组血清IL-8、IL-10、TNF-α、CRP水平较治疗前降低,且联合组低于克霉唑组(P均<0.01);2组阴道pH值、IL-1β水平较治疗前降低,LDH、sIgA水平较治疗前升高,且联合组降低或升高的幅度大于克霉唑组(P均<0.01)。联合组与克霉唑组不良反应总发生率比较差异无统计学意义(3.41%vs. 2.78%,χ^(2)=0.052,P=0.819)。结论乳酸菌阴道胶囊联合克霉唑阴道片治疗妊娠期霉菌性阴道炎效果肯定,可进一步促进患者临床症状缓解和阴道炎性反应控制,调节阴道微生态平衡,且安全性较高。 Objective To observe the clinical effect of lacidophilin vaginal capsules combined with clotrimazole vaginal tablets in the treatment of mycotic vaginitis of pregnancy.Methods A total of 160 patients with mycotic vaginitis of pregnancy admitted to the department of obstetrics and gynecology of the 909 Hospital(Southeast Hospital of Xiamen University)from March 2021 to April 2023 were retrospectively selected and divided into the combination group(88 cases)and the clotrimazole group(72 cases)according to different treatment methods.The clotrimazole group was treated with clotrimazole vaginal tablets,and the combination group was treated with lacidophilin vaginal capsules on the basis of clotrimazole group.The total effective rate,clinical symptom relief time,serum inflammatory factors(IL-8,IL-10,TNF-α,CRP)levels,vaginal microecological indicators(pH,IL-1β,LDH,sIgA)changes before treatment and 1 week after drug withdrawal,and adverse reactions were compared between the two groups.Results The total effective rate of the combination group was 95.45%,which was higher than 83.33%of the clotrimazole group(χ^(2)=6.465,P=0.011).The relief time of abnormal leucorrhea,vaginal pruritus,vaginal mucosal congestion and vaginal pain in the combination group were shorter than those in the clotrimazole group(P<0.01).One week after drug withdrawal,serum IL-8,IL-10,TNF-α,CRP levels were lower than those before treatment,and those in the combination group were lower than those of clotrimazole group(P<0.01).Vaginal pH,IL-1βwere lower than those before treatment,the levels of LDH and sIgA were higher than those before treatment,and the amplitude of decrease or increase in the combination group was greater than that in the clotrimazole group(P<0.01).There was no significant difference in the total incidence of adverse reactions between the combination group and the clotrimazole group(3.41%vs.2.78%,χ^(2)=0.052,P=0.819).Conclusion Effect of lacidophilin vaginal capsules combined with clotrimazole vaginal tablets in the treatment of
