摘要
该文总结分析夏仲元教授“辨体-病证症结合”论治桥本氏病相关经验。夏教授认为辨体需关注甲功、过敏体质;其中脾虚为重要病机,临证采用扶正消瘿八法治疗。甲亢期以养阴清热、重镇安神为主,佐以软坚散结,多用左归丸或知柏地黄丸加减。甲功正常期治以疏肝理气,化痰消瘿,采用柴胡疏肝散合消瘰丸加减。甲减期以补气温阳为先,方药采用金匮肾气丸合二仙汤加减。辨症常用山慈菇、射干、夏枯草、海藻玉壶汤治疗瘿肿;补中益气汤治疗乏力、肌肉关节痛;当归芍药散或瓜蒌薤白半夏汤治疗黏液性水肿。
This article summarized and analyzed professor XIA Zhongyuan's experience in the treatment of Hashimoto's dis-ease.Professor XIA thinks that body differentiation needs to pay attention to thyroid function and allergic constitution.Among them,spleen deficiency is an important pathogenesis,and eight methods for reinforcing healthy Qi and eliminating goiter were used to treat the syndrome.The hyperthyroidism period is mainly to nourish Yin,clear heat,calm the nerves,accompanied by softening hardness and dissipating mass,and the treatment often uses modified Zuogui Pill(左归丸)or modified Zhibai Dihuang Pill(知柏地黄丸).During the period of normal thyroid function,the treatment involves soothingliver and regulating Qi,resolving phlegm and eliminating goiter,and using Chaihu Shugan Powder(柴胡疏肝散)and Xiaoluo Pill(消瘰丸)with modifications.In the hy-pothyroidism stage,the priority is to replenish Qi and warm Yangand the prescription is modified Jingui Shenqi Pill(金匮肾气丸)and Erxian Decoction(二仙汤).According to symptom differentiation,the treatment commonly uses Shancigu(Cremastrae Pseudobulbus Pleiones Pseudobulbus),Shegan(Belamcandae Rhizoma),Xiakucao(Prunellae Spica)and Haizao Yuhu Decoction(海藻玉壶汤)to treat goiter and swelling.Buzhong Yiqi Decoction(补中益气汤)treats fatigue and muscle and joint pain.Dan-ggui Shaoyao Powder(当归芍药散)or Gualou Xiebai Banxia Decoction(瓜蒌薤白半夏汤)for the treatment of mucinous edema.
