摘要
目的:探讨生物反馈训练在直肠癌保肛术后的应用效果。方法:选择2020年2月至2023年2月我院收治的80例直肠癌患者,均采用保肛手术治疗,将其随机分为对照组和观察组,每组40例。对照组患者采用盆底肌锻炼,观察组患者采用生物反馈训练,共干预2个月。干预前后采用中文版纪念斯隆-凯特林癌症中心(MSKCC)肠道功能问卷进行肠道功能评估,检测肛肠动力学指标,比较2组患者前切除综合征发生率。结果:干预前2组患者受饮食影响、便频便急、排便感觉障碍及MSKCC总分比较差异无统计学意义(P>0.05),干预后2组患者排便受饮食影响、便频便急、排便感觉障碍以及MSKCC总分均较干预前升高,且干预后观察组各项评分显著高于对照组(P<0.05);干预前2组患者肛管静息压、肛管最大收缩压、直肠便意感觉容量、直肠最大耐受容量比较差异无统计学意义(P>0.05),干预后2组患者肛管静息压、肛管最大收缩压、直肠便意感觉容量、直肠最大耐受容量均优于干预前,且观察组优于对照组(P<0.05);观察组患者前切除综合征发生率显著低于对照组(P<0.05)。结论:生物反馈训练能够提升直肠癌保肛术后直肠及肛门功能,降低前切除综合征的发生率。
Objective To investigate the effect of biofeedback training in rectal cancer after sphincterpreserving operation.Methods Total 80 patients with rectal cancer admitted to our hospital from February 2020 to February 2023 were selected and treated with sphincter-preserving operation.They were randomly divided into the control group and the observation group,with 40 patients in each group.The control group received pelvic floor muscle exercise,and the observation group received biofeedback training.The intervention lasted for 2 months.The intestinal function questionnaire of Chinese version of Memorial Sloan-Kettering Cancer Center(MSKCC)was used to evaluate intestinal function before and after intervention.Anorectal dynamics indexes were detected,and the incidence of anterior resection syndrome was compared between the two groups.Results Before intervention,there were no significant differences in bowel movement influenced by diet,frequency and urgency of bowel movement,sensory disturbance of bowel movement and total score of MSKCC between the two groups(P>0.05).After intervention,bowel movement influenced by diet,frequency and urgency of bowel movement,sensory disturbance of bowel movement and total score of MSKCC in both groups were increased,and the scores were higher in the observation group than in the control group(P<o.05).Before intervention,there were no significant differences in the anal resting pressure and maximal contraction pressure,rectal defecation sensation and maximum tolerable volume between the two groups(P>0.05).After intervention,anal resting pressure and maximal contraction pressure,rectal defecation sensation and maximum tolerable volume in the two groups were better than before intervention,and was better in the observation group than in the control group(P<0.05).The incidence of anterior excision syndrome in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Biofeedback training can improve the function of rectum and anus after sphincter-preservi
