盐酸右美托咪定对手术全身麻醉高血压患者气管拔管期心血管反应的临床疗效

Clinical Effect of Dexmedetomidine Hydrochloride on Cardiovascular Response in Patients with General Anesthesia and Hypertension during Tracheal Extubation

摘要 目的评价术中全身麻醉高血压患者于气管拔管期行盐酸右美托咪定方案的效果及对心血管反应的影响。方法选取2019年1月—2022年12月河北省秦皇岛市海港医院全身麻醉高血压患者1300例,随机数表法分为对照组和研究组,各650例,对照组行生理盐水方案,研究组采取盐酸右美托咪定方案,比较两组拔管情况、心血管反应、不良事件发生率。结果研究组拔管时镇静评分高于对照组,疼痛评分低于对照组,差异有统计学意义(P<0.05);拔管5 min后研究组平均动脉压、心率均低于对照组,差异有统计学意义(P<0.05);研究组不良反应总发生率为0.93%,低于对照组,差异有统计学意义(χ^(2)=8.510,P<0.05)。结论全身麻醉高血压患者以盐酸右美托咪定药物进行辅助治疗时,可以减小气管拔管期的疼痛程度,有良好镇静作用,心血管反应平稳,不良反应发生率下降。 Objective To evaluate the effect of dexmedetomidine hydrochloride on cardiovascular response in patients with general anesthesia and hypertension during tracheal extubation.Methods A total of 1300 hypertensive patients under general anesthesia were selected from Harbor Hospital of Qinhuangdao City,Hebei Province from January 2019 to December 2022,and were divided into control group and study group by random number table method,650 cases in each group.The control group received normal saline regimen,and the study group received dexmedetomidine hydrochloride regimen.The extubation status,cardiovascular reactions and incidence of adverse events were compared between the two groups.Results The sedation score of the study group was higher than that of the control group,and the pain score was lower than that of the control group,and the difference was statistically significant(P<0.05).5 min after extubation,the mean arterial pressure and heart rate of the study group were lower than those of the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the study group was 0.93%,lower than that in the control group,and the difference was statistically significant(χ^(2)=8.510,P<0.05).Conclusion When patients with general anesthesia and hypertension are treated with dexmedetomidine hydrochloride as an adjuvant,it can reduce the pain level during tracheal extubation,have a good sedative effect,have a stable cardiovascular response,and reduce the incidence of adverse reactions.
