摘要
目的:探究老年高血压手术患者全身麻醉苏醒拔管期采用小剂量盐酸右美托咪定的临床效果。方法:2018年7月-2020年7月收治老年高血压手术患者106例,随机分为两组,各53例。试验组全身麻醉苏醒拔管期采用小剂量盐酸右美托咪定泵注,对照组采用0.9%氯化钠注射液等体积泵注,比较两组临床应用效果。结果:试验组T1期(泵注10 min后)、T2期(拔管即刻)的心率(HR)及平均动脉压(MAP)水平升高幅度均显著低于对照组,差异有统计学意义(P<0.05);试验组HR、MAP水平T3期(拔管后5 min)趋于正常,对照组HR、MAP水平在T3期均高于试验组,差异有统计学意义(P<0.05);试验组麻醉期间不良反应发生率显著低于对照组,差异有统计学意义(P<0.05)。结论:对老年高血压手术患者全身麻醉苏醒拔管期应用小剂量盐酸右美托咪定,患者心率、平均动脉压恢复较为平稳,术后不良反应发生率较低。
Objective:To explore the clinical effect of low dose dexmedetomidine hydrochloride in extubation period of general anesthesia in elderly patients with hypertension surgery.Methods:From July 2018 to July 2020,106 elderly patients with hypertension were selected and randomly divided into two groups with 53 cases in each group.In the experimental group,small dose of dexmedetomidine hydrochloride was injected by pump during extubation.The control group was given equal volume 0.9%sodium chloride injection by pump.The clinical effects of the two groups were compared.Results:In the experimental group,the heart rate(HR)and mean arterial pressure(MAP)in T1(10 min after pump injection)and T2(immediately after extubation)were significantly lower than those in the control group,the difference was statistically significant(P<0.05).In the experimental group,the levels of HR and MAP tended to be normal at T3(5 min after extubation),while in the control group,the levels of HR and MAP were higher than those in the experimental group at T3,the difference was statistically significant(P<0.05).In the experimental group,the incidence of adverse reactions during anesthesia was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The application of low dose dexmedetomidine hydrochloride in the extubation period of general anesthesia in elderly patients with hypertension during operation can stabilize the recovery of heart rate and mean arterial pressure,and reduce the incidence of postoperative adverse reactions.
