地佐辛联合右美托咪定对老年高血压患者全麻苏醒期躁动及血流动力学的影响 被引量:4

Effects of dezocine combined with dexmedetomidine on agitation and hemodynamics during recovery from general anesthesia in elderly hypertensive patients

摘要 目的探讨地佐辛联合右美托咪定对老年高血压患者全麻苏醒期躁动及血流动力学的影响。方法选取收治的100例行全麻手术治疗的老年高血压患者,分为对照组和观察组,各50例,给予对照组患者常规药物芬太尼,给予观察组患者地佐辛联合右美托咪定镇静,记录2组患者手术时间、自主呼吸恢复时间、苏醒时间、拔管时间,比较不同时点的血流动力学水平、疼痛、镇静评分、苏醒期间躁动程度、高血压及心动过缓发生率。结果2组患者手术时间、自主呼吸恢复时间、苏醒时间、拔管时间差异均无统计学意义(P>0.05),与对照组相比,观察组T1~T3时SBP和DBP显著降低,心率明显减慢,观察组T1~T2时MAP也显著增加(P<0.05);与对照组相比,观察组拔管后30 min以及拔管后60 min的视觉模拟评分(VAS)显著降低,Ramsay镇静评分显著升高,但拔管即刻2组比较差异有统计学意义(P<0.05),与对照组相比,观察组躁动程度发生率显著降低(P<0.05),心动过缓发生率差异无统计学意义(P>0.05)。结论地佐辛联合右美托咪定可有效改善老年高血压患者全麻苏醒期躁动,稳定血流动力学水平,提高患者的苏醒质量。 Objective To investigate the effect of dezocine combined with dexmedetomidine on agitation and hemodynamics during the recovery period of general anesthesia in elderly patients with hypertension.Methods A total of 100 elderly patients with hypertension who underwent general anesthesia surgery in our hospital were selected as the research objects.According to random number table method,they were divided into control group and observation group,with 50 cases in each group.Patients in the control group were given conventional drug fentanyl,and patients in the observation group were sedated by dezocine combined with dexmedetomidine.The operation time,recovery time of spontaneous breathing,recovery time and extubation time of the two groups were recorded.The hemodynamic level,pain,sedation score,degree of agitation during recovery,incidence of hypertension and bradycardia were compared at different time points.Results There was no significant difference in operation time,spontaneous breathing recovery time,wake-up time,and extubation time between the two groups(P>0.05).Compared with the control group,the SBP and DBP of the observation group were significantly decreased from T1 to T3,and the HR was significantly decreased.In the observation group,MAP also increased significantly at T1-T2(P<0.05);the Visual Analogue Scale(VAS)at 30 minutes and at 60 minutes after extubation was significantly decreased,and the Ramsay sedation score was significantly increased.However,there was a statistically significant difference between the two groups only immediately after extubation(P<0.05).Compared with the control group,the incidence of agitation in the observation group was significantly lower(P<0.05),and there was no significant difference in the incidence of bradycardia(P>0.05).Conclusion The combination of dezocine and dexmedetomidine can effectively improve the restlessness,stabilize the hemodynamic level and improve the recovery quality of elderly patients with hypertension during the recovery of general anesthesia.
