摘要
目的:探讨基于失效模式与效应分析(FMEA)下的风险管理对消化内镜消毒质量的影响。方法:选取2022年1—12月深圳市龙岗区人民医院消化内镜中心所用的消化内镜(含胃镜和肠镜)90件作为研究对象,其中以2022年1—6月未开展基于FMEA下的风险管理为实施前,2022年7—12月实施基于FMEA下的风险管理为实施后。比较实施前后主要失效模式的优先风险系数(RPN)和消化内镜消毒合格率。结果:经过FMEA风险管理的4项失效模式RPN值均下降到<100分;总体RPN值由756分下降到507分,RPN值下降率为67.06%。实施后消化内镜消毒合格率高于FMEA实施前,差异有统计学意义(P=0.018)。结论:基于FMEA下的风险管理可提高消化内镜消毒合格率,优化消化内镜清洗消毒的工作流程,提高消化内镜消毒管理质量。
Objective:To explore the impact of risk management based on failure mode and effect analysis(FMEA)on the quality of digestive endoscopy sterilization.Methods:Ninety pieces of digestive endoscopes(including gastroscope and enteroscopy)used in the Digestive Endoscopy Center of Shenzhen Longgang District People's Hospital from January to December 2022 were selected as the study object.Among them,January-June 2022 without risk management under FMEA as pre-implementation,and July-December 2022 with risk management under FMEA as post-implementation.Priority risk factor(RPN)for major failure modes and digestive endoscopy sterilization pass rate were compared before and after implementation.Results:the RPN values for all four failure modes after FMEA risk management were reduced to<100 points;The overall RPN value decreased from 756 to 507 points,and the decrease rate of RPN value was 67.06%;The qualified rate of digestive endoscopy sterilization after implementation was higher than that before FMEA implementation,and the difference was statistically significant(P=0.018).Conclusion:Risk management based on FMEA can improve the qualified rate of digestive endoscopy disinfection,optimize the workflow of digestive endoscopy cleaning and disinfection,and improve the quality of digestive endoscopy disinfection management.
