摘要
目的 探索基于医疗失效模式与效应分析的危急值处置流程对护士危急值纵向分析能力的影响。方法 组建危急值处置流程的项目研究团队,运用医疗失效模式与效应分析对医院目前的住院患者危急值报告和处置流程进行风险评估,找出失效模式,分析失效原因,并结合CCU的专科特点制订针对性的改进方案并实施,最后进行效果评价。结果 应用医疗失效模式与效应分析进行改进后,护理人员的专科危急值及危急值处置流程的知晓情况、护士的危急值规范处置执行情况均明显提高(P<0.05),实施后RPN值明显改善。结论 基于医疗失效模式与效应分析的危急值处置流程能够有效提高护士的专科危急值及处置流程的知晓率及执行率,进而提高CCU护士对危急值纵向分析能力,保证患者的安全。
Objective To explore the influence of critical value disposal process based on medical failure mode and effect analysis on nurses'longitudinal critical value analysis ability.Methods A project research team of critical value disposal process was set up to conduct risk assessment on the current"critical value reporting and disposal process for inpatients"in the hospital by using medical failure mode and effect analysis,to find out the failure mode and analyze the failure causes,and to formulate and implement targeted improvement plans based on the specialty characteristics of CCU,and finally to evaluate the effect.Results After the application of HFMEA,the nurses'knowledge of the specialized critical value and the critical value disposal process,and the nurses'implementation of the standardized critical value disposal were significantly improved(P<0.05),and the RPN value was significantly improved after the implementation.Conclusions The critical value management procedure based on HFEMA model can effectively improve the nurses'specialized critical value and the awareness rate and implementation rate of the management procedure,so as to improve the longitudinal analysis ability of CCU nurses to ensure the safety of patients.
