摘要
目的探讨甘肃省各市州医疗卫生机构运行效率及环境因素的影响,为有效提高医疗卫生机构运行效率提供参考。方法应用三阶段DEA模型和Malmquist模型分析框架,选取2017年—2020年甘肃省14个市州医疗卫生机构相关的面板数据进行实证分析。结果经过三阶段DEA分析后,综合效率值、纯技术效率值分别上升0.3%、2.1%,规模效率值降低1.6%,综合效率始终有效的地区有张掖、定西、陇南;动态效率测量中,2017年—2020年甘肃省医疗机构全要素生产率年均降低3.3%,其中技术进步指数年均降低5.1%,是影响全要素生产率下降的主要因素。结论甘肃省各市州医疗机构运行效率存在明显的地区差异,外部环境影响医疗机构运行效率;建议因地制宜,提高医疗技术水平,优化外部环境,引导医疗服务高质量发展。
Objective To explore the operational efficiency of medical and health care institutions in Gansu Province and its influencing factors.Methods By using a three-stage data envelope analysis(DEA)model and the Malmquist Index,we selected the panel data of medical and health care institutions in 14 cities and prefectures in Gansu Province from 2017 to 2020 for empirical analysis.Results After the three-stage DEA,the values of integrated efficiency and pure technical efficiency increased by 0.3%and 2.1%,respectively,whereas the value of scale efficiency decreased by 1.6%.The integrated effciency values remained effective in Zhangye,Dingxi,and Longnan.In the dynamic efficiency measurements,the total factor productivity decreased by an average of 3.3%per annum in the period from 2017 to 2020,of which the"technical progress"decreased by 5.1%per year on average,becoming the main factor contributing to the decline of total factor productivity.Conclusions The operational efficiency of medical and health care institutions differs dramatically among cities and prefectures in Gansu Province and is affected by external environments.Tailored policies should be created and implemented in various regions to enhance the healthcare capacity,optimize external environments,and promote the high-quality development of medical services.
