摘要
目的探究婴幼儿听性脑干反应(ABR)、短纯音听性脑干反应(tb-ABR)与行为测听(BA)的相关性及其诊断价值。方法采用回顾性分析法,选择2021年10月至2022年10月于南京市妇幼保健院进行检查的80例婴幼儿,对其进行ABR、tb-ABR、BA测试,依据ABR阈值测试结果将婴幼儿分为4组,A组(≤35 dB nHL,30侧耳),B组(>35~50 dB nHL,64侧耳),C组(>50~70 dB nHL,46侧耳),D组(>70~90 dB nHL,9侧耳)。分析各组ABR与各频率tb-ABR、BA阈值的相关性,以及各组ABR阈值与各频率tb-ABR的相关性。结果各组的tb-ABR的2、4 kHz频率阈值与click-ABR阈值均呈正相关(P<0.05),且A组和C组随频率升高相关系数有逐渐上升趋势。A组的1~4 kHz及B组和C组的click-ABR阈值与BA各频率阈值均呈正相关(P<0.05),且A组随频率升高相关系数有逐渐上升趋势。A组和B组的1 kHz,B组和C组的0.5 kHz,各组2、4 kHz的tb-ABR阈值与BA相同频率阈值均呈正相关(P<0.05),且A组及D组的相关系数有随频率升高逐渐上升趋势。结论婴幼儿的ABR、tb-ABR与BA的相关性较高,可对婴幼儿听力功能进行更加综合完善的评估。
Objective To investigate the correlation of auditory brainstem response(ABR),tone burst auditory brainstem response(tb-ABR),and behavioral audiometry(BA)and its diagnostic value.Methods Using retrospective analysis,80 infants and toddlers who were examined at Nanjing Maternal and Child Health Hospital from October 2021 to October 2022 were selected and tested for ABR,tb-ABR,and BA,and the infants and toddlers were divided into 4 groups based on the results of ABR threshold test,group A(≤35 dB nHL,30 ears),group B(>35-50 dB nHL,64 ears),group C(>50-70 dB nHL,46 ears),and group D(>70-90 dB nHL,9 ears).The correlation between the ABR of each group and the threshold of each frequency tb-ABR and BA,and the correlation between the threshold of each group and the frequency tb-ABR were analyzed.Results The 2 and 4 kHz frequency thresholds of tb-ABR in each group were positively correlated with the click-ABR thresholds,and the correlation coefficients of groups A and C tended to increase gradually with increasing frequency.1-4 kHz in group A and click-ABR thresholds in groups B and C were positively correlated with each frequency threshold of BA(P<0.05),and the correlation coefficients of group A tended to increase gradually with increasing frequency.The tb-ABR thresholds at 1 kHz in groups A and B,0.5 kHz in groups B and C,and 2 and 4 kHz in each group were positively correlated with the same frequency thresholds of BA(P<0.05),and the correlation coefficients of groups A and D tended to increase gradually with increasing frequency.Conclusion The ABR and tb-ABR of infants have high correlation with BA,which can be used for a more comprehensive and perfect evaluation of infant hearing function.
