摘要
目的:探讨C反应蛋白(CRP)在早期急性上呼吸道感染中的诊疗价值。方法:回顾性分析2020年12月—2023年4月于柳州市人民医院门诊、急诊就诊的260例早期急性上呼吸道感染患者临床资料,采用成组设计,以CRP不高不用抗生素治疗患者为对照组(A组),共65例;以CRP升高用抗生素治疗患者为观察组(B组),共62例。两组治疗14d内进行电话随访,统计治愈时间。采用一减累积生存分析函数分析两组治愈率,采用散点图和Spearman秩相关分析CRP与血白细胞(WBC)的相关性。结果:两组治愈时间比较,差异无统计学意义(P>0.05);一减累积生存分析函数治愈曲线显示,两组治愈率交叉并且几乎重叠,差异无统计意义(P>0.05);CRP与WBC相关分析显示,260例早期急性上呼吸道感染患者CRP与WBC水平呈弱正相关(r=0.13,P<0.05)。结论:急性上呼吸道感染中,以CRP升高判定为细菌感染并使用抗生素治疗的效果一般,CRP在急性上呼吸道感染诊疗中的指导价值较弱。
Objective To investigate the diagnosis and treatment value of C-reactive prtein(CRP)in the early acute upper respiratory tract infection.Methods Clinical data of 260 patients with early acute upper respiratory tract infection treated in outpatient department and emergency department of Liuzhou People’s Hospital from December 2020 to April 2023 were retrospectively analyzed.Group design was adopted,65 patients with low CRP and no antibiotics were selected as control group(Group A).The observation group(Group B)included 62 patients with elevated CRP treated with antibiotics.The two groups were followed up by telephone within 14 days of treatment to count the cure time.The cure rate of the two groups was analyzed by one-minus survival analysis function,and the correlation between CRP and WBC was analyzed by scatter chart and Spearman rank correlation analysis.Results There was no significant difference in healing time between the two groups(P>0.05).The one-minus survival analysis function cure curve showed that the cure rates of the two groups crossed and almost overlapped,with no statistical significance(P>0.05).Correlation analysis between CRP and WBC showed that there was a weak positive correlation between CRP and WBC level in 260 patients with early acute upper respiratory tract infection(r=0.13,P<0.05).Conclusions In acute upper respiratory tract infection,the effect of bacterial infection with elevated CRP and antibiotic treatment is not good,and the guiding value of CRP in the diagnosis and treatment of acute upper respiratory tract infection is weak.
