摘要
目的探究临床应用心肺超声对急性左心力衰竭伴肺水肿患者无创正压通气临床结局的预测价值。方法对医院在2021年7月-2022年6月期间收治的70例行诊断治疗的急性左心衰伴肺水肿患者作为研究对象,随机将其分为对照组与观察组,对照组患者35例,应用常规药物治疗;观察组患者35例,在药物治疗基础上给予无创正压通气治疗,两组均行心肺超声诊断,比较两组患者临床疗效,并分析心肺超声在预测患者临床结局方面的价值。结果对照组患者治疗总有效率82.9%,观察组患者总有效率94.3%,对照组患者治疗总有效率低于观察组,两组对比有统计意义(P<0.05);治疗前两组患者的心肺超声评分差异无统计学意义(P>0.05),治疗后两组患者的心肺超声评分均降低,并且治疗后观察组患者的心肺超声评分低于对照组,差异均具有统计学意义(P<0.05);根据观察组患者的临床结局,将其分为治疗成功组(33例)与治疗失败组(2例),发现两组患者左室射血分数(EF)、二尖瓣环平面收缩期位移(MAPSE)指标对比差异不明显,而三尖瓣最大反流速度VTR、右心室Sm、二尖瓣E/e改为S’、二尖瓣E/e’PASP指标以及肺部超声评分中治疗成功组与治疗失败组间存在差异,有统计学意义(P<0.05)。结论在临床对急性左心力衰竭伴肺水肿患者检查中,应用心肺超声检查,可预测患者无创正压通气治疗临床结局,发挥应用价值。
Objective To investigate the clinical value of cardiopulmonary ultrasound in predicting the clinical outcome of noninvasive positive pressure ventilation in patients with acute left heart failure and pulmonary edema.Methods Retrospective analysis was made on 70 patients with acute left heart failure and pulmonary edema who were diagnosed and treated in the hospital from July 2021 to June 2022.They were randomly divided into the control group and the observation group.35 patients in the control group were treated with conventional drugs;35 patients in the observation group were treated with non-invasive positive pressure ventilation on the basis of drug treatment.The two groups were diagnosed with cardiopulmonary ultrasound.The clinical efficacy of the two groups was compared,and the value of cardiopulmonary ultrasound in predicting the clinical outcome of patients was analyzed.Results The total effective rate of the patients in the control group was 82.9%,and the total effective rate of the patients in the observation group was 94.3%.The total effective rate of the patients in the control group was lower than that in the observation group,which was statistically significant(P<0.05);There was no significant difference between the two groups in cardiopulmonary ultrasound scores before treatment(P>0.05).After treatment,the cardiopulmonary ultrasound scores of patients in both groups decreased,and the cardiopulmonary ultrasound scores of patients in the observation group after treatment were lower than those in the control group after intervention.The differences were statistically significant(P<0.05);According to the clinical outcome of the patients of the observation group,they were divided into the treatment success group(33 cases)and the treatment failure group(2 cases).It was found that there was no significant difference in EF and MAPSE indicators between the two groups,but there were significant differences in VTR,S’,mitral valve E/e',PASP indicators and pulmonary ultrasound scores between the treatment succ
