摘要
目的:分析细节护理干预对血液透析尿毒症患者生命质量及睡眠质量的影响。方法:选取2021年8月至2023年2月明溪县总医院血透室收治的血液透析尿毒症患者70例作为研究对象,按照随机数字表法分为观察组和对照组,每组35例。对照组给予透析护理干预,观察组给予细节护理干预。采用匹兹堡睡眠质量指数(PSQI)比较2组患者干预前后睡眠质量的变化;采用瘙痒视觉模拟量表比较2组患者的瘙痒症状改善效果;采用自我管理能力量表(ESCA)比较2组患者的自我管理能力的变化;采用GQOLI-74量表比较2组患者的生命质量;比较2组患者营养相关指标的变化,包括白蛋白、血红蛋白、转铁蛋白;比较血液透析尿毒症患者血钙、甲状旁腺激素水平,并比较2组患者心律失常、低血压、高血压等并发症发生情况,同时比较2组患者护理满意度。结果:护理干预下,观察组血液透析尿毒症患者白蛋白、血红蛋白、转铁蛋白水平显著高于对照组,观察组患者护理满意度显著高于对照组,观察组患者血钙水平高于对照组,甲状旁腺激素水平显著低于对照组,观察组瘙痒视觉模拟量表评分、PSQI量表评分显著低于对照组,观察组自我管理ESCA量表评分、GQOLI-74量表评分显著高于对照组,观察组患者心律失常、低血压、高血压总发生率显著低于对照组,差异均有统计学意义(均P<0.05)。结论:细节护理干预对血液透析尿毒症患者生命质量及睡眠质量具有积极影响,能够有效提高患者的自我管理效能,提高患者的生命质量,降低患者的心律失常、低血压、高血压的发生率,升高血钙水平,降低甲状旁腺激素水平,有效改善瘙痒症状,进而提高患者的睡眠质量,值得临床推广使用。
Objective:To analyze the effects of detailed nursing intervention on quality of life and sleep in patients with hemodialysis uremia.Methods:A total of 70 patients with hemodialysis uremia admitted to the hemodialysis room of Mingxi County General Hospital from August 2021 to February 2023 were selected as the research objects and were divided into observation group and control group according to the random number table method,with 35 cases in each group.The control group was given dialysis nursing intervention,and the observation group was given detailed nursing intervention.Pittsburgh Sleep Quality Index(PSQI)was used to compare the changes of sleep quality before and after the intervention.The effect of itch symptom improvement in 2 groups was compared by Visual Analogue Scale.Exercise of Self-care Agency(ESCA)scale was used to compare the changes of self-management ability between the 2 groups.The quality of life of 2 groups was compared by Generic Quality of Life Inventory-74(GQOLI-74)scale.The changes of nutrition-related indexes,including albumin,hemoglobin and transferrin,were compared between the two groups.The levels of blood calcium and parathyroid hormone in patients with hemodialysis uremia were compared,the occurrence of complications such as arrhythmia,hypotension and hypertension were compared between the two groups,and the nursing satisfaction of patients was compared as well.Results:Under nursing intervention,the levels of albumin,hemoglobin and transferrin of hemodialysis uremia patients in the observation group were significantly higher than those of the control group,the nursing satisfaction of patients in the observation group was significantly higher than the control group,the blood calcium level of patients in the observation group was higher than the control group,the level of parathyroid hormone was significantly lower than the control group,and the scores of itch visual analog scale and PSQI scale in the observation group were significantly lower than the control group.The ESCA scale sco
