摘要
目的对尿毒症患者进行血液灌流联合碳酸镧治疗并分析其对皮肤瘙痒、钙磷代谢情况的影响。方法便利选取2021年1月—2022年12月南通市通州区人民医院肾脏内科规律透析的尿毒症患者82例,按照治疗方法分为观察组(41例、血液灌流+碳酸镧)与对照组(41例、血液灌流),比较两组皮肤瘙痒情况、钙磷代谢情况、肾脏功能水平、临床疗效及生活质量。结果观察组的5-D瘙痒量表评分显著低于对照组,差异有统计学意义(P<0.05);治疗后观察组的Ca显著高于对照组,P、PTH、SCr及BUN低于对照组,差异有统计学意义(P<0.05);观察组总有效率为95.12%,高于对照组的78.05%,差异有统计学意义(χ^(2)=5.145,P<0.05);治疗后观察组多项SF-36因子评分均显著提高,差异有统计学意义(P<0.05)。结论尿毒症患者在接受血液灌流、碳酸镧联合治疗后,疗效更显著,皮肤瘙痒症状得以缓解,钙磷代谢得以纠正,肾功能得以改善,生活质量得以提高。
Objective To investigate the effects of hemoperfusion combined with lanthanum carbonate therapy on skin itching and calcium and phosphorus metabolism in patients with uremia.Methods A total of 82 uremia patients with regular dialysis in the Department of Renal Medicine of Tongzhou District People′s Hospital of Nantong City from January 2021 to December 2022 were conveniently selected and divided into the observation group(41 cases with hemoplavage+lanthanum carbonate)and the control group(41 cases with hemoplavage)according to treatment methods.The skin pruritosis,calcium and phosphorus metabolism,kidney function,clinical efficacy and quality of life of the two groups were compared.Results The 5-D pruritus scale score in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).After treatment,Ca in the observation group was significantly higher than that in the control group,while P,PTH,SCr and BUN were lower than that in the control group,and the difference was statistically significant(P<0.05).The total effective rate of the observation group was 95.12%,higher than that of the control group(78.05%),and the difference was statistically significant(χ^(2)=5.145,P<0.05).The scores of multiple SF-36 factors in the observation group were significantly higher after treatment,and the difference was statistically significant(P<0.05).Conclusion After receiving hemoperfusion and lanthanum carbonate combined treatment for uremia patients,the curative effect is more significant.The symptoms of skin itching are relieved,the metabolism of calcium and phosphorus is corrected,the renal function is improved,and the quality of life is improved.
