摘要
目的:探讨微创经皮钢板内固定术(MIPPO)与切开复位内固定术(ORIF)治疗胫骨平台粉碎性骨折的疗效。方法:选择2019年1月~2020年10月本院收治的90例胫骨平台粉碎性骨折,按随机数字表法将患者分为对照组(45例)和观察组(45例)。对照组行ORIF,观察组行MIPPO。比较两组围术期相关指标、膝关节功能恢复情况和康复效果。结果:观察组手术时间、骨折愈合时间、功能恢复时间短于对照组,术中出血量低于对照组,有统计学差异(P<0.05);两组术前活动度、疼痛、稳定性评分相比,无统计学差异(P>0.05),术后3个月两组活动度、疼痛、稳定性评分均高于术前,且观察组各评分高于对照组,有统计学差异(P<0.05);观察组康复总有效率高于对照组,有统计学差异(P<0.05)。结论:与ORIF相比,使用MIPPO治疗胫骨平台粉碎性骨折效果更佳,其能够缩短手术用时,加快骨折愈合和功能恢复,改善膝关节功,提高康复效果。
Objective:To investigate the efficacy of minimally invasive percutaneous locking plate internal fixation(MIPPO)and open reduction internal fixation(ORIF)in the treatment of comminuted fractures of the tibial plateau.Methods:A total of 90 patients with comminuted tibial plateau fractures admitted to our hospital from January 2019 to October 2020 were selected and divided into control group(45 cases)and observation group(45 cases)according to random number table method.ORIF for control group and MIPPO for observation group.The perioperative related indexes,the recovery of knee function and the rehabilitation effect were compared between the two groups.Results:The operative time,fracture healing time and functional recovery time of the observation group were shorter than those of the control group,and the amount of intraoperative blood loss was lower than that of the control group,with statistical difference(P<0.05).There were no statistically significant differences in activity,pain and stability scores between the two groups before surgery(P>0.05).3 months after surgery,activity,pain and stability scores of the two groups were higher than before surgery,and the scores of the observation group were higher than the control group,with statistical differences(P<0.05).The total effective rate of rehabilitation in the observation group was higher than that in the control group,with statistical difference(P<0.05).Conclusion:Compared with ORIF,MIPPO is more effective in the treatment of comminuted fracture of tibial plateau.MIPPO can shorten the operation time,accelerate fracture healing and functional recovery,improve knee joint function and improve rehabilitation effect.
