等速肌力训练联合早期部分负重训练对胫骨平台骨折术后患者肌力及膝关节功能的影响 被引量:1

Effects of Isokinetic Muscle Strength Training Combined with Early Partial Weight-Bearing Training on Muscle Strength and Knee Joint Function in Patients after Tibial Plateau Fracture Surgery

摘要 目的探讨等速肌力训练联合早期部分负重训练对胫骨平台骨折术后患者肌力及膝关节功能的影响.方法选取2020年4月—2022年4月于本院诊治的64胫骨平台骨折患者为研究对象,以随机数表法分为两组.两组均行胫骨内固定术,术后对照组(n=32)接受常规康复训练,观察组(n=32)接受等速肌力训练联合早期部分负重训练,均连续训练3个月.比较两组疼痛程度、膝关节功能、下肢肌力.结果干预后,观察组疼痛视觉模拟评分为(1.57±0.26)分,低于对照组的(3.48±0.51)分,差异有统计学意义(P<0.05);观察组美国纽约特种外科医院膝关节功能评分量表中疼痛评分(22.19±2.25)分、功能评分(15.61±2.29)分、活动度评分(13.17±2.11)分、肌力评分(7.23±0.86)分、屈曲畸形评分(7.49±0.63)分、稳定性评分(7.13±0.51)分,均高于对照组的(20.26±2.11)分、(13.38±2.23)分、(11.15±2.07)分、(5.16±0.78)分、(5.23±0.56)分、(5.35±0.48)分,组间差异有统计学意义(P<0.05);观察组徒手肌力测试法评分为(5.31±0.27)分,高于对照组的(3.25±0.19)分,差异有统计学意义(P<0.05).结论对胫骨平台骨折术后患者实施等速肌力训练与早期部分负重训练,可有效减轻疼痛,恢复膝关节功能,改善下肢肌力. Objective To explore the effect of isokinetic muscle strength training combined with early partial weightbearing training on muscle strength and knee joint function in patients after tibial plateau fracture surgery.Methods 64 patients with tibial plateau fracture who were treated in the hospital from April 2020 to April 2022 were selected as the research objects and divided into two groups by random number table.Both groups were treated with tibial internal fixation surgery.After surgery,the control group(n=32)received routine rehabilitation training,and the observation group(n=32)received isokinetic muscle strength training combined with early partial weight-bearing training.Both groups were trained continuously for 3 months.Pain degree,knee joint function and lower limb muscle strength were compared between the two groups.Results After intervention,the pain visual analogue scale score of the observation group was(1.57±0.26)points,which was lower than(3.48±0.51)points of the control group,and the difference was statistically significant(P<0.05);the scores of pain,function,range of motion,muscle strength,flexion deformity and stability of hospital for special surgery knee joint score in the observation group were(22.19±2.25)points,(15.61±2.29)points,(13.17±2.11)points,(7.23±0.86)points,(7.49±0.63)points,(7.13±0.51)points,which were higher than(20.26±2.11)points,(13.38±2.23)points,(11.15±2.07)points,(5.16±0.78)points,(5.23±0.56)points,(5.35±0.48)points of the control group,and the differences between the groups were statistically significant(P<0.05);the manual muscle testing score of the observation group was(5.31±0.27)points,which was higher than(3.25±0.19)points of the control group,and the difference was statistically significant(P<0.05).Conclusion Isokinetic muscle strength training and early partial weight-bearing training for patients after tibial plateau fracture surgery can effectively relieve pain,recover knee joint function and improve lower limb muscle strength.
