摘要
目的拟对15项重症肌无力生活质量(MG-QOL15)量表汉语版在中国成人MG患者临床评估中的应用进行信度、效度和灵敏度的评估。方法纳入183例临床确诊的中国成人MG患者,所有患者均进行MG-QOL15、MG日常生活量表(ADL)、徒手肌力量表(MMT)和改良的美国重症肌无力基金会-疾病严重程度量表(MGFA-QMG)评分。采用SPSS19.0统计软件进行统计学分析,Cronbachα系数评价量表的内部一致性,用因子分析法评价量表的结构信度,用Spearman相关系数评价校标关联效度和量表之间的相关性。其中77例患者于6个月后进行随访,对前后量表评分分值进行Wilcoxon singal检验以验证灵敏度。结果 MG-QOL15量表的Cronbachα系数为0.911,提示该量表内部一致性好。MG-QOL15量表有3项公因子,累积方差值为64.883%,提示量表的结构效度良好。MG-QOL15量表与MG-QMG和MG-ADL的相关系数分别为0.540、0.638,均为中度相关。症状改善组的45例患者的MGQOL15量表ES值>0.5,提示灵敏度较高。结论 MG-QOL15量表的内部一致性和结构信度良好,MG-QOL15量表与其他量表具有中度相关性;在病情改善的MG患者中,该量表的灵敏度较高,可有效用于中国成人MG患者的临床评估。
Aim The 15-item Myasthenia Gravis Quality of Life（MG-QOL15） questionnaire is a common instrument for evaluation of myasthenia gravis（MG） associated disability. This study aim was to assess the validity, reliability,and sensitivity of Chinese version in the Chinese adult MG patients. Methods 183 Chinese adult patients with MG were enrolled. All the patients completed the Chinese MGQOL15 and MG-specific activities of daily living（ADL）, Manual Muscle test（MMT）, the quantitative MG score of the Myasthenia Gravis Foundation of America（MGFA-QMG）. Data was analyzed by using SPSS 19.0. Internal consistency of the scales was assessed with Cronbach α. The construct validity was assessedwith factor analysis,criterion validity was assessed by spearman correlation analysis. 77 patients were followed up 6 months after baseline visit. And varied sensitivity was assessed with Wilcoxon signed-rank test. Results The MG-QOL15 showed good internal consistency. The Cronbach α is 0.911. Factor analysis showed good construct validity（accumulative dedication rate 64.883%）. The correlation coefficients between MGQOL15 and MG-QMG, MG-ADL scores were 0.540, 0.638 respectively（P〈0.001）, which showed higher sensitivity to symptom improvement group. Conclusion The Chinese version of MGQOL15 is a valid and reliable questionnaire for determination of quality of life in Chinese adult MG patients. There was moderate correlation between MG-QOL15 and other three scales, and it is effective for clinical evaluation of adult MG patients in China.
