摘要
目的 探讨四川省某大型三级甲等医院医疗组长层级抗菌药物使用强度(Antibiotics Use Density,AUD)与病例组合指数(Case Mix Index,CMI)的相关性,为实现公立医院AUD的精细化管理提供实证依据。方法 运用灰色关联分析法探讨医疗组长层级AUD与CMI的相关性,并对比分析精细化管理的实施效果。结果 医疗组长及其所在亚专业CMI与AUD关联系数分别为0.96和0.94,共361名医疗组长CMI与AUD的关联系数大于0.9。精细化管理实施后,截至2022年5月,全院AUD降至34.58,医疗组长AUD下降幅度前5位的灰色关联系数均小于0.9,最低仅为0.754。结论 CMI是AUD的重要影响因素,应重点关注高AUD、低CMI的医疗组长,实现公立医院抗菌药物的医院层级、科室层级和医疗组长层级的精细化管理。
Objective To explore the correlation between Antibiotics Use Density(AUD) and Case Mix Index(CMI) on the medical group-level in a tertiary three comprehensive hospital in Sichuan Province and to provide empirical basis for the refined management of AUD in public hospitals.Methods Grey Relational Analysis was used to evaluate the association degree between AUD and CMI on the medical group-level,and comparative analysis method was used to evaluate the effect of the refined management of AUD.Results The gray relation coefficient between the CMI and AUD of the medical group and its clinical sub-groups were 0.96 and 0.94 respectively.A total of 361 medical team leaders had a CMI and AUD correlation coefficient greater than 0.9.Following the implementation of precision management,the hospital-wide AUD dropped to 34.58 as of May 2022,with the top 5 decreases in medical team leader AUD having a grey-off contact number of less than 0.9,with the lowest being only 0.754.Conclusion CMI is an important influencing factor for AUD.More attentions should be paid to these medical groups with lower CMI and higher AUD to achieve refined management at hospital level,department level and medical team leader level of antimicrobial drugs in public hospitals.
