摘要
目的分析四川省名中医梁翼治疗膝关节骨性关节炎中药运用规律,为临床用药的中药客观化分类与中医诊疗提供参考。方法选取四川省骨科医院名中医梁翼门诊2021年4月至2022年4月诊疗膝关节骨性关节炎350例患者的处方,统计并分析单味中药的四气五味、归经、功效等,选取使用频次≥41次的高频中药,采用SPSS 23.0统计学软件进行分层聚类分析。结果共使用中药44味。其中,中药四气多为温(61.36%)、平(18.18%)、寒(15.91%),五味多为甘(40.91%)、苦(27.27%)、辛(22.73%),归经多为肝(45.45%)、脾(20.45%)、肾(13.64%)。使用频次最高的中药为熟地黄、当归和川牛膝。使用频次≥41次的中药40味,占总用药频次的91.04%(5538/6083);根据高频聚类分析树状图可聚为3类。结论四川省名中医梁翼以“筋骨并重、骨正筋顺”为治疗原则,以补益肝肾、活血化瘀辨证施治,多应用补虚、活血化瘀和祛风湿药,以及疏肝健脾、补肾祛风活络药,起到筋骨并治、骨强筋柔、消肿止痛之功效。
Objective To analyze the application rules of traditional Chinese medicines(TCM)in the treatment of knee osteoarthritis by Liang Yi(a famous TCM doctor in Sichuan Province),and to provide a reference for the TCM objective classification and TCM diagnosis and treatment in the clinical medication.Methods The prescription data of 350 patients with knee osteoarthritis diagnosed and treated at the Liang Yi famous TCM outpatient department in the Sichuan Province Orthopedic Hospital from April 2021 to April 2022 were selected,and the four natures and five flavors,meridian tropism,and efficacy of single TCM were statistically analyzed.The high-frequency TCM with a use frequency of≥41 times was selected for hierarchical clustering analysis by the SPSS 23.0 software.Results A total of 44 TCM were used.Among them,the four natures of TCM were mainly warm(61.36%),neutral(18.18%),and cold(15.91%),the five flavors were mainly sweet(40.91%),bitter(27.27%),and spicy(22.73%),and the meridian tropism were mainly liver(45.45%),spleen(20.45%),and kidney(13.64%).The most frequently used TCM were Rehmanniae Radix Praeparata,Angelicae Sinensis Radix,and Radix Achyranthis Bidentatae.There were 40 TCM with a use frequency of≥41 times,accounting for 91.04%(5538/6083)of the total use frequency,and they were classified into three categories according to the tree view of the high-frequency clustering analysis.Conclusion Liang Yi,a famous TCM doctor in Sichuan Province,treats the disease according to the treatment principle of″reinforcing both bones and tendons,and strengthening bones and softening tendons″,and treats it according to the syndrome differentiation of tonifying the liver and kidney,promoting blood circulation and removing blood stasis.He often treats the disease by TCM with the efficacy of tonifying deficiency,promoting blood circulation and removing blood stasis,dispelling wind and dampness,soothing the liver,strengthening the spleen,tonifying the kidney,dispelling wind and activating collaterals,which have the effects
