摘要
近年,随着《糖尿病周围神经病变中医临床诊疗指南(2016年版)》(以下简称《指南》)的推出,标志着中医对该病的诊疗逐渐规范。在此基础上,首先对该病的发生发展及瘀血、痰湿的产生进行了探讨;并参考《指南》的辨证分型,进行治疗用药的整理,其中对气虚血瘀证、阴虚血瘀证、阳虚寒凝证、肝肾精亏证的治疗,应用《指南》推荐方即取效显著,且通过临床观察及实验室研究,对影响推荐方疗效的因素进行了探讨,值得参考;对于痰瘀阻络证、湿热瘀阻证的治疗,因其病机复杂,则以各家论治为主,多从脾肾论治,疗效尚可;同时针对该病经络阻滞的特点,临床常合用通经活络药以达到治疗效果,故将此类药进行分类整理,为随证用药提供思路。
In recent years,with the introduction of the Guidelines for the Clinical Diagnosis and Treatment of Diabetic Peripheral Neuropathy(2016 edition)(hereinafter referred to as the Guidelines),it marks the gradual standardization of the diagnosis and treatment of the disease in traditional Chinese medicine. On this basis,the occurrence and development of the disease and the production of blood stasis and phlegm dampness were first expounded,and the treatment of drugs was arranged with reference to the syndrome differentiation and classification of the Guidelines,among which the treatment of Qi deficiency and blood stasis,Yin deficiency and blood stasis,Yang deficiency and cold coagulation,liver and kidney essence deficiency syndrome were treated. Through clinical observation and laboratory research, the factors influencing the efficacy of the recommended prescription were discussed, which is worthy of reference. Treatment of phlegm stasis and obstruction of collaterals syndrome,dampness and heat stasis syndrome,because of its complex pathogenesis,is based on the treatment of each family,more from the spleen and kidney treatment,the curative effect is acceptable;At the same time,according to the characteristics of the disease meridian block,clinical use of meridian activating drugs to achieve therapeutic effect,so the classification of such drugs to provide ideas for the use of drugs with syndrome.
