摘要
目的:观察中药复方"当归四逆汤"及甲钴胺片治疗糖尿病周围神经病变(寒凝血瘀型)的临床优效性研究,为中医药指导治疗糖尿病周围神经病变提供有力证据,拓宽治疗思路和方法。方法:采用随机对照及前瞻性观察的试验方法,筛选出符合诊断标准、纳入标准的糖尿病周围神经病变(寒凝血瘀型)患者60例,用随机数字表方法,随机分为两个组(治疗组和对照组),两组在糖尿病控制血糖基础治疗上,治疗组给予中药汤剂"当归四逆汤",对照组给予甲钴胺片(每次500μg,日3次餐后口服),疗程8周,分别观察治疗组及对照组治疗前后的中医临床证候评分、上下肢神经传导速度变化。将所得观察数据,分别采用t检验及χ2检验,进行统计学分析。结果:经过中药复方汤剂"当归四逆汤"治疗后,治疗组测定患者上肢的正中神经、下肢的腓总神经、胫神经传导速度,显示均较治疗前有显著改善(P<0.05);对照组经过甲钴胺片口服治疗后,患者的上肢正中神经、下肢的腓总神经、胫神经运动神经传导速度,以及上肢的正中神经感觉神经传导速度,均较治疗前,有明显改善(P<0.05),对照组患者的下肢腓总神经及下肢的胫神经感觉神经传导速度测定较治疗前改善无统计学意义。治疗与对照组间比较,针对下肢的腓总神经及下肢的胫神经传导速度均有改善明显,有统计学意义(P<0.05),针对患者上肢的正中神经传导速度测定的改善程度,两组间比较无显著差异。治疗组患者在改善临床症状方面,中医证候评分优于对照组患者(P<0.05)。结论:中药复方汤剂"当归四逆汤"可提高糖尿病周围神经病变(寒凝血瘀型)下肢的腓总神经、下肢的胫神经传导速度、显著改善患者临床症状,其临床疗效优于甲钴胺片组,针对上肢的正中神经传导速度同样改善明显,与甲钴胺组疗效相当。
Objective ： To observe the efficiency of Danggui Sini Decoction and Methycobal in the treatment of cold congealing and blood stasis syndrome of type 2 diabetes peripheral neuropathy and provide the important ideas and methods. Method：By prospective randomized controlled method, 60 patients with type 2 diabetes peripheral neuropathy were randomly divided into treatment group and control group. Bedsides the treatment for lowering blood glucose in two groups, the treatment group was treated with Danggui Sini Decoction and the control group was given Methycobal 500 μg orally three times a day. After 8 weeks treatment, the changes of TCM syndrome score and nerve conduction velocity were observed in the two groups. The statistical analysis of the data was carried out by t test and X2 test. Result：The median nerve, the nerve conduction velocity of the nerve and the tibial nerve conduction velocity were improved in the treatment group （ P 〈 0. 05）. In the control group, the nerve conduction velocity of median nerve, the nerve conduction velocity of the tibial nerve and the nerve conduction velocity of the median nerve were improved （ P 〈 0. 05 ）. There was no significant difference between the treatment group and the control group and the difference was statistically significant （ P 〈 0.05 ）. The difference was statistically significant. The treatment group was superior to the control group （ P 〈 0. 05 ）. Conclusion ：Danggui Sini Decoction can improve the common peroneal nerve and tibial nerve conduction velocity of cold congealing and blood stasis syndrome of diabetic peripheral neuropathy and significantly improve the clinical symptoms. The curative effect is better than that of MI group. The median nerve conduction velocity was also significantly improved, with the equivalent efficacy of Mecobalamin.
