摘要
骨关节炎(osteoarthritis, OA)是以关节软骨退行性改变和关节疼痛为特征的一种慢性骨关节疾病,严重时可出现关节活动障碍,多发于中老年人群。随着现代社会人口老龄化加剧,骨关节炎呈现逐年递增的趋势。miRNA作为骨关节炎的循环生物标志物,其异常表达可为早期诊断骨关节炎提供依据。许多miRNA被证实参与软骨细胞的增殖分化、凋亡、自噬及细胞外基质(extracellular matrix, ECM)降解,并与骨关节炎涉及的多种信号通路相互调节。现代研究表明,一些补益肝肾、活血化瘀类中药通过激活信号调节器miRNA,使其与mRNA的特定序列结合转录后调控基因表达,抑制炎性反应、延缓软骨细胞衰老,起到防治作用。阐述了骨关节炎的病理机制及其与miRNA的相关性,归纳总结中医药基于miRNA对骨关节炎的调节保护作用,旨在为中医药治疗骨关节炎提供一条新的作用途径。
Osteoarthritis(OA)is a chronic osteoarthritis characterised by degenerative changes in joint cartilage and joint pain.In severe cases,joint mobility disorders can occur,mostly in middle-aged and elderly people.With the aging of the population in modern society,osteoarthritis shows an increasing trend year by year.As a circulating biomarker of osteoarthritis,miRNA abnormal expression can provide a basis for early diagnosis of osteoarthritis.Many miRNAs have been confirmed to be involved in chondrocyte proliferation and differentiation,apoptosis,autophagy and extracellular matrix degradation,and are regulated with multiple signaling pathways involved in osteoarthritis.Modern research shows that some traditional Chinese medicines that can nourish liver and kidney,promote blood circulation and remove blood stasis by activating the signal regulator miRNA,bind it to a specific sequence of mRNA and regulate gene expression after transcription,inhibit inflammatory reactions,delay the ageing of chondrocytes,and play a preventive role.This paper described the pathological mechanism of osteoarthritis and its correlation with miRNA.It summarized the regulatory and protection effect of traditional Chinese medicine on osteoarthritis based on miRNA,aiming to provide a new way for traditional Chinese medicine treating osteoarthritis.
