血清25-羟维生素D_(3)和T淋巴细胞亚群与儿童哮喘发病风险和控制水平的关系 被引量:3

Association of serum 25-hydroxyvitamin D_(3) and T lymphocyte subsets with risk and control level of childhood asthma

摘要 目的 探讨血清25-羟维生素D3[25-(OH)D3]和T淋巴细胞亚群与儿童哮喘发病风险和控制水平的关系。方法 选择2020年5月—2022年4月收治的83例哮喘患儿作为研究对象(哮喘组),另外选择50例健康体检儿童作为健康组,比较2组儿童25-(OH)D3和T淋巴细胞亚群水平;根据哮喘控制水平分级标准将患儿分为良好控制组53例、部分控制组19例、未控制组11例,比较不同控制水平患儿的25-(OH)D3和T淋巴细胞亚群水平,分析25-(OH)D_(3)与T淋巴细胞亚群水平的关系。结果 哮喘组患儿25-(OH)D_(3)、CD3、CD4、CD4/CD8、NK细胞低于健康组(P<0.05);25-(OH)D_(3)、CD3、CD4、CD4/CD8是哮喘的主要影响因素(P<0.05);未控制组和部分控制组患儿25-(OH)D_(3)、CD3、CD4、CD4/CD8、NK细胞水平低于良好控制组(P<0.05),且未控制组患儿水平低于部分控制组(P<0.05);健康儿童25-(OH)D3与T淋巴细胞亚群均无相关性(P>0.05),哮喘儿童25-(OH)D3水平与CD3、CD4、CD4/CD8成正相关(P<0.05)。结论 哮喘儿童25-(OH)D_(3)和T淋巴细胞亚群水平低下,且二者存在显著相关性,哮喘的发生和病情控制不佳可能与缺乏维生素D导致免疫功能低下有关,哮喘儿童可适当补充维生素D。 Objective This paper aims to investigate the association of serum 25-hydroxyvitamin D_(3)[25-(OH)D_(3)] and T lymphocyte subsets with risk and control level of childhood asthma. Methods A total of 83 children with asthma admitted from May 2020 to April 2022 were selected as subjects(asthma group). In addition, 50 healthy children were selected as the healthy group, and the levels of 25-(OH)D3and T lymphocyte subsets were compared between the two groups. According to the classification criteria of asthma control level, the children were divided into good control group(n=53), partial control group(n=19) and non-control group(n=11). The levels of 25-(OH)D_(3)and T lymphocyte subsets of the children with different control levels were compared. The correlation between 25-(OH)D3and T lymphocyte subsets in asthmatic children was analyzed. Results The levels of 25-(OH)D3, CD3, CD4, CD4/CD8 and NK cells in asthma group were significantly lower than those in healthy group(P<0.05). 25-(OH)D_(3), CD3, CD4, CD4/CD8 are risk factors for asthma(P<0.05). The levels of 25-(OH)D_(3), CD3, CD4, CD4/CD8 and NK cells in non-control group and partial control group were lower than those in good control group(P<0.05), and the level of non-control group was lower than that of partial control group(P<0.05). There was no correlation between 25-(OH)D3and T lymphocyte subsets in healthy children(P>0.05). The level of 25-(OH)D3in asthma children was positively correlated with CD3, CD4, CD4/CD8(P<0.05). Conclusion The levels of 25-(OH)D3and T lymphocyte subsets are low in asthma children, and there is a significant correlation between 25-(OH)D3and T lymphocyte subsets. The occurrence and poor control of asthma may be related to the immune deficiency caused by vitamin D deficiency, and vitamin D supplementation should be appropriately performed on asthma children.
