阴道微生态变化与人乳头瘤病毒感染及宫颈病变的关系 被引量:2

摘要 目的探讨阴道微生态变化与人乳头瘤病毒(HPV)感染及宫颈低级别鳞状上皮内病变的关系。方法选取2021年1月至2022年5月在北京市昌平区霍营社区卫生服务中心诊断为宫颈低级别鳞状上皮内病变的116例患者为研究组,选择同期116名健康体检女性为对照组,两组均进行HPV-DNA检测及阴道微生态检测,并比较两组的检测结果。结果研究组的乳酸杆菌检出率低于对照组,研究组的加德纳菌、白细胞酯酶阳性、唾液酸苷酶、阴道pH值≥4.6检出率高于对照组,差异有统计学意义(P<0.05)。研究组的HPV感染率高于对照组,HPV未感染率低于对照组,差异有统计学意义(P<0.05)。HPV感染者的乳酸杆菌检出率低于HPV未感染者,HPV感染者的加德纳菌、白细胞酯酶阳性、唾液酸苷酶、阴道pH值≥4.6检出率高于HPV未感染者,差异有统计学意义(P<0.05)。结论阴道微生态变化与HPV感染、宫颈低级别鳞状上皮内病变存在一定关系,阴道微生态检测可辅助临床早期识别HPV感染、宫颈低级别鳞状上皮内病变。 Objective To investigate the relationship between vaginal microecological changes and human papilloma virus(HPV)infection and cervical low-grade squamous intraepithelial lesion.Methods A total of 116 patients diagnosed with cervical low-grade squamous intraepithelial lesion in Huoying Community Health Service Center of Changping District in Beijing from January 2021 to May 2022 were selected as the study group,and 116 healthy women in the same period were selected as the control group.Both groups were tested for HPV-DNA and vaginal microecological,and the test results of the two groups were compared.Results The detection rate of Lactobacillus in the study group was lower than that in the control group,and the detection rates of Gardnerella,leucocyte esterase,sialidase and vaginal pH≥4.6 in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).The HPV infection rate in the study group was higher than that in the control group,and the HPV non-infection rate was lower than that in the control group,and the differences were statistically significant(P<0.05).The detection rate of Lactobacillus in HPV infected patients was lower than that in HPV uninfected patients,the detection rates of Gardnerella,leucocyte esterase,sialidase and vaginal pH≥4.6 in HPV infected patients were higher than those in HPV uninfected patients,and the differences were statistically significant(P<0.05).Conclusion The changes of vaginal microecological are related to HPV infection and cervical low-grade squamous intraepithelial lesion.The detection of vaginal microecological can assist in early clinical identification of HPV infection and cervical low-grade squamous intraepithelial lesion.
