摘要
目的观察健脾利湿解毒汤治疗宫颈HR-HPV感染(脾虚湿盛型)的临床疗效及对阴道微环境的影响。方法选择2017年1月—2018年12月医院收治的宫颈HR-HPV感染患者196例,依据随机对照法将患者分为研究组和对照组,每组98例。对照组采用重组人干扰素α2b治疗,在停经后3 d,睡前将重组人干扰素α2b放置阴道深处,1次/d。研究组在对照组治疗的基础上,口服健脾利湿解毒汤治疗。连续治疗3个月经周期,每个月经周期治疗14 d。比较两组治疗前后中医证候积分变化,比较治疗总有效率,比较两组治疗后阴道微生态恢复情况,比较两组治疗前后菌群密集度和菌群多样性变化。结果研究组治疗总有效率高于对照组(P<0.05);研究组治疗后中医证候积分低于对照组(P<0.05);研究组治疗后阴道微生态恢复情况优于对照组(P<0.05);两组治疗后菌群密集程度较治疗前有所改善,研究组改善幅度优于对照组(P<0.05);两组治疗后菌群多样性较治疗前有所改善,研究组改善幅度优于对照组(P<0.05)。结论健脾利湿解毒汤结合重组人干扰素α2b栓治疗宫颈HR-HPV感染疗效确切,可更大程度改善患者的临床症状,改善阴道菌群密集度和多样性,促进阴道微生态恢复,提高治疗疗效,值得进一步研究探讨。
Objective To observe the clinical efficacy of Jianpi Lishi Jiedu Decoction(健脾利湿解毒汤)in the treatment of HR-HPV infection of the cervix(spleen deficiency and dampness type)and its effect on the vaginal microenvironment.Methods A total of 196 patients with cervical HR-HPV infection admitted to the hospital from January 2017 to December 2018 were selected.According to the randomized control method,the patients were divided into study group and control group,with 98 patients in each group.The control group was treated with recombinant human interferon alpha 2B.Three days after menopause,the recombinant human interferonα2b was placed deep in the vagina once per day before sleep.On the basis of the treatment in the control group,the study group was treated by Jianpi Lishi Jiedu Decoction orally.Three menstrual cycles were treated consecutively,and each cycle was treated for 14 days.The changes of TCM symptom scores before and after treatment were compared between the two groups.The total effective rate of treatment was compared.The recovery of vaginal microecology after treatment was compared between the two groups,and the changes of bacterial density and bacterial diversity before and after treatment were compared between the two groups.Results The total effective rate of the treatment group was higher than that of the control group(P<0.05).The score of TCM syndromes in the study group was lower than that in the control group(P<0.05).The recovery of vaginal microecology in the study group was better than that in the control group(P<0.05).The concentration of bacterial flora in the two groups after treatment was improved compared with that before treatment,and the improvement of the study group was better than that of the control group(P<0.05).After treatment,the bacterial diversity of the two groups was improved,and the improvement of the study group was better than that of the control group(P<0.05).Conclusion The combination of Jianpi Lishi Jidu Decoction and recombinant human interferonα2b in the trea
