摘要
目的分析高危型人乳头瘤病毒(high-Risk human papillomavirus,HR-HPV)感染者的阴道微生态平衡状态,探讨微生态失衡对HR-HPV感染及其相关宫颈病变的作用及其相互关联。方法以640例HR-HPV感染者(HR-HPV感染组)为研究对象,依随访结果分为持续感染组和非持续感染组,按宫颈上皮内瘤病变(cervical intraepithelial neoplasia,CIN)(宫颈病变组)程度分为CINⅠ组、CINⅡ组、CINⅢ组、宫颈癌组,采用聚合酶链式反应(polymerase chain reaction,PCR)对HR-HPV进行基因分型检测,显微镜镜检阴道微生物群的形态变化,评价微生态体系。结果①细菌性阴道病(bacterial vaginosis,BV)阳性率在宫颈癌和CIN间存在差异(P<0.05)。②阴道微生态失衡与HR-HPV持续感染合并的宫颈病变无关(r=0.143,P>0.05),与持续性感染相关(r=0.114,P<0.05)。结论阴道微生态失衡增加了HR-HPV的易感性,促进宫颈上皮内瘤病变形成,进而导致宫颈癌的发生。
Objective To analyze the status of vaginal microecological balance in High-risk human papillomavirus(HR-HPV)patients,and to explore the effect of microecological imbalance on HR-HPV infection and related cervical lesions and their correlation.Methods 640 patients with HR-HPV infection were divided into persistent infection group and non-persistent infection group according to the results of follow-up visit.According to the degree of Cervical intraepithelial neoplasia(CIN),the patients were divided into CINⅠgroup,CINⅡgroup,CINⅢgroup and Cervical cancer group.Genotyping of HR-HPV was performed by polymerase chain reaction(PCR),microscopic changes of vaginal microbiota were observed and micro-ecological system was evaluated by microscopy.Results①The positive rate of bacterial vaginosis(BV)was different between cervical cancer and CIN(P<0.05).②Vaginal microecological imbalance had nothing to do with persistent HR-HPV infection combined with cervical lesions(r=0.143,P>0.05),but was related to persistent infection(r=0.114,P<0.05).Conclusion Vaginal microecological imbalance increases HR-HPV susceptibility,promotes the formation of cervical intraepithelial neoplasia,and then leads to the occurrence of cervical cancer.
