摘要
目的评价单侧颞下颌关节病(TMD)患者患病侧与对侧无症状颞下颌关节的核磁共振(MRI)影像结构异常之间的关系。方法采用MRI对颞下颌关节疾病判断的金标准,对249例有单侧TMD症状的患者进行研究,检查两侧颞下颌关节(TMJ)的关节盘位置及形态、关节骨髓腔信号及是否存在骨关节炎(OA)、关节积液。结果有症状侧关节盘移位及形变、骨关节炎、关节积液、骨髓腔信号异常较无症状侧明显(P<0.05),关节积液随病程时间延长呈加重趋势(P<0.05)。有症状侧单侧可复性颞下颌关节盘前移位(DDWR)发生率和无症状侧比较,差异无统计学意义(P>0.05),不可复性关节盘前移(DDWOR)发生率高于无症状侧(P<0.05)。逐步逻辑回归分析显示,无症状侧除DDWR之外的任何MRI异常的存在与患侧的骨关节炎和关节盘形变有关(P<0.05),无症状侧MRI异常仅与病变侧存在骨关节炎有关(P<0.05)。结论单侧有症状TMD与对侧无症状的MRI结构异常有关,提示有症状侧TMD进展和无症状侧TMJ变化的发生存在关联性。
Objective To investigate the correlation between unilateral temporomandibular joint disorder(TMD)and contralateral asymptomatic temporomandibular joint structural abnormalities by MRI.Methods A total of 249 patients with the symptoms of unilateral TMD were enrolled in the study.Taking MRI examination results as gold standard,the position of articular disc and deformation,joint bone marrow cavity signal,osteoarthritis,hydrops articuli in both temporomandibular joints(TMJ)were detected.Results The disc displacement and deformation,abnormal signal in bone marrowcavity,osteoarthritis,hydrops articuli on symptomatic side were more obvious than those on asymptomatic side(P<0.05).The joint effusion was aggravated with the duration of the disease.However there was no significant difference in the incidence of disease types of disc displacement with reduce(DDWR)between the two sides(P>0.05).Stepwise logistic regression showed that the presence of any imaging abnormality except for DDWR on symptomatic side was related to disc deformation and osteoarthritis,however,which on asymptomatic side was related to the osteoarthritis only.Conclusion The unilateral symptomatic TMD is related to the MRI structural abnomalities on the contralateral asymptomatic side,which suggests that the progression of joint changes on symptomatic side is correlated with the chandes of TMJ on asymptomatic side.
