摘要
目的研究应用头颈联合线圈和专用咬合器在磁共振检查颞下颌关节紊乱病中的成像效果。方法选取2019年5月—2020年5月该院收治的60例TMD患者,分析60例颞下颌关节紊乱病患者120侧颞下颌关节的MRI结果。结果120侧颞下颌关节均获得清晰的图像,其中52侧正常盘髁关系,53侧关节盘形态改变(均为不可复性盘前移患者),22侧髁突骨质异常(不可复性关节盘前移位中伴髁突骨质异常的比例为42%,显著高于18%的伴髁突骨质异常的可复性关节盘前移位的比例),提示随着关节盘移位及形变进一步进展,出现关节退行性改变风险增加。结论应用头颈联合线圈和专用咬合器拍摄的颞下颌关节磁共振图像能清楚地显示关节盘形态、位置及骨质异常的变化,在颞下颌关节紊乱病的检查和诊断方面具有重要价值。
Objective To study the imaging effect of combined head and neck coil and special articulator in MRI examination of temporomandibular joint disorders.Methods 60 patients with TMD treated in the hospital from May 2019 to may 2020 were selected to analyze the MRI resu1ts of 120 temporomandibular joints in 60 patients with temporomandibular disorders were analyzed.Results Clear images were obtained in 120 temporomanclibular joints,52 normal disc condylar relationship,53 joints changed in shape(all were patients with irreversible disc advancement),and 22 had abnormal condylar bone(the rate of irreversible anterior disc displacement with condylar bone abnormalities was 42%,which was significantly higher than the 18% rate of reducible anterior disc displacements with condylar bone abnormalities).It was suggested that with the further progress of articular disc displacement and deformation,the risk of joint degenerative changes increases.Conclusion The magnetic resonance image of temporomandibular joint taken by head and neck combined coil and special articulator can clearly show the changes of joint disc shape,position and bone abnormality,which is of great value in the examination and diagnosis of temporomandibular joint disorders.
