多层面多维度MRI评估颞下颌关节盘位置的价值 被引量:2

The value of multi-slice and multi-dimensional MRI in evaluating the position of temporomandibular joint disc

摘要 目的在颞下颌关节MRI图像中比较颞下颌关节紊乱病(TMD)诊断最常用的正中斜矢状位评估法和多层面斜矢状位及斜冠状位共同评估法对颞下颌关节盘移位的诊断价值。方法收集162侧符合DC/TMD临床诊断标准的颞下颌关节MRI图像,使用两种方法评估颞下颌关节盘的位置。方法一是在斜矢状面的正中层面评估关节盘位置;方法二则需在连续图层的斜矢状和斜冠状图像上评估关节盘位置。使用Pearsonχ^(2)检验评价两种方法的差异。结果正中斜矢状位影像评估发现139侧(85.8%)关节盘位置改变,而多层斜矢状位和冠状位影像共同评估发现151侧(93.2%)关节盘位置改变。方法二灵敏度显著高于方法一(P<0.05)。结论多层面多维度的MRI评估颞下颌关节盘位置优于单层面,可以有效避免假阴性诊断,对颞下颌关节紊乱病具有较高的诊断价值。 Objective To compare the diagnostic value of the most commonly used median oblique sagittal evaluation method and multi-slice oblique sagittal and oblique coronal evaluation methods for temporomandibular joint(TMJ)disc displacement in TMJ MRI images.Methods The MRI images of 162 sides of temporomandibular joint meeting the clinical diagnostic criteria of DC/TMD were collected,and the two methods were used to evaluate the position of TMJ disc.The first method was to evaluate the disc position on the oblique sagittal scans of the central slice only;while the second method is to evaluate all the oblique sagittal and oblique coronal scans.The Pearson′s chi-square test was used to evaluate the differences between the two methods.Results Median oblique sagittal imaging identified disc displacement in 139 sides(85.8%),while multi-slice oblique sagittal and coronal imaging combined identified disc displacement in 151 sides(93.2%).The sensitivity of the second method was significantly higher than that of the first method(P<0.05).Conclusion Multi-slice and multi-dimensional MRI is better than single level in evaluating the position of temporomandibular joint disc,which can effectively avoid false negative diagnosis and has high diagnostic value for temporomandibular joint disorders.
