摘要
目的系统评估精子端粒长度(STL)作为诊断男性不育症生物标志物的准确性与临床价值。方法计算机检索Pubmed、Cochrane图书馆、Web of Science、Embase、Scopus、ScienceDirect、万方数据库及中国知网(CNKI)从建库至2022年1月STL与男性不育症的相关性研究。数据提取由两名审查员独立进行,采用Review Manager 5.3、IBM SPSS Statistics 25.0进行统计学分析。结果本研究最终共10项研究符合纳入标准,其中482例生育男性和442例不育男性。研究发现生育男性同不育男性的精子总数(WMD 2.73;95%CI:1.70-3.76)、前向运动精子百分率(a+b)%(WMD2.70;95%CI:1.67-3.72)、精子浓度(WMD 2.77;95%CI:1.52-4.02)和STL(WMD 1.81;95%CI:1.18-2.45)水平相比,均明显较高,且STL与精液参数存在显著线性正相关。受试者工作特征曲线(ROC)表明STL诊断男性不育症的最佳截断值(cut-off值)为1.0,敏感度和特异度均为80%。结论STL与男性不育症有显著相关性,且在检测精子生育功能方面比常规精液参数有更高的准确性,可作为临床诊断男性不育症新的生物标志物。
Objective To systematically evaluate the accuracy and clinical value of sperm telomere length(STL)as a biomarker for the diagnosis of male infertility.Methods The correlation studies between STL and male infertility were searched from the database of Pubmed,Cochrane Library,Web of Science,Embase,Scopus,ScienceDirect,Wanfang database and China Knowledge Network(CNKI)from the establishment to January 2022.Data extraction was performed independently by two reviewers and statistical analysis was performed using Review Manager 5.3,IBM SPSS Statistics 25.0.Results A total of 10 studies which met the inclusion criteria for this study,including 482 fertile and 442 infertile men were obtained.Compared with those of infertile men,sperm count(WMD 2.73;95%CI:1.70-3.76),percentage of forward-moving sperm(a+b)%(WMD 2.70;95%CI:1.67-3.72),sperm concentration(WMD 2.77;95%CI:1.52-4.02),and STL(WMD 1.81;95%CI:1.18-2.45)of fertile men were significantly increased.There was a significant positive linear correlation between STL and semen parameters.The ROC(Receiver Operating Characteristic)curve indicated that the optimal cut-off value of STL for the diagnosis of male infertility was 1.0,with a sensitivity and specificity of 80%.Conclusions STL is significantly correlated with male infertility.It has higher accuracy in evaluating sperm fertility function than conventional semen parameters,and can be used as a new biomarker for the diagnosis of male infertility.
