摘要
目的分析育龄期精液B族链球菌(GBS)定植特点及其对精液主要参数的影响。方法收集该院生殖中心2019年1月至2021年12月人工辅助受精精液标本,进行培养分离和药物敏感试验,同时进行精液常规检查。结果4480例精液标本检出GBS 369株,定植率为8.24%,2019,2020,2021年定植率分别是5.80%、7.44%和10.63%,<30岁、30~40岁和>40岁定植率分别是5.62%、8.50%和11.31%。369株GBS对青霉素、氨苄青霉素、利奈唑胺、奎奴/达福普汀、万古霉素的耐药率均为0,左氧氟沙星的耐药率为26.56%,莫西沙星的耐药率为24.12%,红霉素素的耐药率为94.65%,四环素的耐药率为95.17%,克林霉素的耐药率为96.23%。GBS定植组精子密度、精子总活力、前向运动精子百分率和正常形态精子显著性低于无GBS定植组。结论精液GBS定植呈逐龄上升趋势,且GBS定植会降低精子质量,干预治疗应重视药敏试验结果合理选用抗菌药物。
Objective To analyze the colonization characteristics of group B Streptococcus(GBS)in semen of reproductive age and its effect on semen parameters.Methods Collected semen specimens from artificial assisted fertilization at the reproductive center from January 2019 to December 2021 for culture separation and drug sensitivity test,at the same time semen routine analysis was performed.Results A total of 369 strains of GBS were detected from 4480 semen,and the colonization rate was 8.24%.The colonization rate of 2019,2020 and 2021 was 5.80%,7.44% and 10.63% respectively.The colonization rate of<30 years,30 to 40 years and>40 years was 5.62%,8.50% and 11.31%respectively.The resistance rate of 369 strains of GBS to penicillin,ampicillin,linezolid,quino/dalfopristin and vancomycin were all 0,and the resistance rate of levofloxacin was 26.56%,moxifloxacin was 24.12%,erythromycin was 94.65%,tetracycline was 95.17%,and clindamycin was 96.23%.The sperm density,the total sperm motility ratio and the forward motile sperm ratio in GBS colonization group were significantly lower than those without GBS colonization.Conclusion The colonization rate of GBS in semen of childbearing age population is increasing age by age,and GBS colonization can reduce sperm quality.The clinical intervention treatment should pay attention to the results of drug susceptibility tests and rationally select antibacterial drugs.
