脑胶质瘤MRI平扫及强化特征分析 被引量:2

摘要 目的:分析不同级别脑胶质瘤MRI(核磁共振)平扫、强化DWI及MRS图像特征。方法:本次研究对象来源于本院及总院2018年9月至2021年6月收治的68例脑胶质瘤患者,均给予MRI检查,对比低级别组、高级别组病灶周边水肿程度、ADC值(表观弥散系数)、NAA/Cr(氢质子波谱)、Cho/Cr(胆碱/肌酸)、Cho/NAA(胆碱/N-乙酰天门冬氨酸)。结果:68例患者经MRI检查:12例I级(17.65%),18例II级(26.47%),21例III级(30.88%),17例IV级(25.00%)。低级别组无水肿率(53.33%)、轻度水肿率(40.00%)高于高级别组(2.63%、34.21%),低级别组中度水肿率(6.67%)、重度水肿率(0)均低于高级别组(31.58%、31.58%),P<0.05(差异均具有统计学意义)。低级别组ADC值高于高级别组,P<0.05(差异均具有统计学意义)。低级别组Cho/Cr、Cho/NAA均低于高级别组,低级别组NAA/Cr高于高级别组,P<0.05(差异均具有统计学意义)。68例患者经MRI检查,15例胶质母细胞瘤(22.06%),17例少突胶质细胞瘤(25.00%),13例混合性胶质瘤(19.12%),9例室管膜瘤(13.24%),4例脉络丛乳头状瘤(5.88%)。结论:MRI可以对脑胶质瘤分级作出准确诊断,为临床治疗提供科学的参考依据。 Objective:To analyze the image features of MRI(nuclear magnetic resonance)plain scan,enhanced DWI and MRS in different grades of glioma.Methods:The subjects of this study were 68 patients with glioma admitted from our hospital and the general hospital from September 2018 to June 2021.All of them were given MRI examination,and the degree of edema surrounding the lesion,ADC value(apparent diffusion coefficient),NAA/Cr(hydrogen proton spectroscopy),CHO/Cr(choline/creatine),CHO/NAA(choline/N-acetylaspartate)were compared between the low-level group and the high-level group.Results:The results of 68 patients by MRI examination shows 12 cases were grade I(17.65%),18 cases were grade I(26.47%),21 cases were grade III(30.88%),17 cases were grade IV(25.00%).The rates of waterless edema(53.33%)and mild edema(40.00%)were higher in the low-level group than those in the high-level group(2.63%and 34.21%),and the rates of moderate edema(6.67%)and severe edema(0)were lower in the low-level group than those in the high-level group(31.58%and 31.58%),P<0.05(the difference was statistically significant).The ADC value of the low-level group was higher than that of the high-level group(P<0.05).Cho/Cr and Cho/NAA were lower in the low-level group than in the high-level group,and NAA/CR was higher in the low-level group than in the high-level group,P<0.05(the difference was statistically significant).The results of 68 patients by MRI examination shows that 15 cases were glioblastomas(22.06%),17 cases were oligodendrogliomas(25.00%),13 cases were mixed gliomas(19.12%),9 cases were ependymoma(13.24%)and 4 cases were choroid plexus papilloma(5.88%).Conclusion:MRI can accurately diagnose the grade of glioma and provide scientific reference for clinical treatment.
