摘要
目的探讨磁共振波谱(MRS)在立体定向活组织检查术诊断脑胶质瘤中的应用价值。方法回顾性分析2012年6月至2016年12月解放军总医院第六医学中心神经外科收治的32例脑胶质瘤患者的临床资料。所有患者均行开颅手术切除肿瘤,术后经病理学检查诊断为胶质瘤。其中,14例采用MRS检查辅助立体定向活组织检查术(试验组),18例仅行常规MRI检查辅助立体定向活组织检查术(对照组)。以开颅手术后确定的病理学诊断结果为标准,通过Fisher精确检验比较两种方法诊断脑胶质瘤的准确率。结果两组患者在年龄、性别、病变影像学特征方面的差异均无统计学意义(均P>0.05)。32例脑胶质瘤患者均顺利完成立体定向活组织检查术,术后无一例发生颅内出血、感染及新发神经功能障碍。试验组的诊断准确比例为13/14,高于对照组的10/18,且两组的差异有统计学意义(P=0.044)。结论应用MRS可明显提高立体定向活组织检查手术诊断脑胶质瘤的准确性,其对活组织检查手术靶点的规划具有指导意义。
Objective To explore the application value of magnetic resonance spectroscopy(MRS)in stereotactic biopsy for the diagnosis of glioma.Methods A retrospective analysis was conducted on the clinical data of 32 patients with glioma who were admitted to Department of Neurosurgery,the Sixth Medical Center of PLA General Hospital from June 2012 to December 2016.All patients underwent craniotomy to remove the tumor,and postoperative pathological examination verified the diagnosis of glioma.In this series,14 cases underwent MRS-assisted stereotactic biopsy of tissues(experimental group)and 18 cases underwent regular MRI-assisted stereotactic biopsy without MRS(control group).Based on the pathological diagnosis determined post craniotomy,Fisher exact test was used to compare the accuracy of diagnosis of glioma between the 2 groups.Results The differences in age,sex and imaging characteristics between the 2 groups were not statistically significant(all P>0.05).All 32 patients with glioma successfully underwent stereotactic biopsy,and none of the cases had intracranial hemorrhage,infection or new neurofunctional disorders post surgery.The ratio of accurate diagnosis in the experimental group was 13/14,which was higher than 10/18 in the control group,and the difference between the 2 groups was statistically significant(P=0.044).Conclusion The application of MRS can obviously improve the accuracy of stereotactic biopsy for the diagnosis of glioma,which is instructive to the planning of surgical target of stereotactic biopsy.
