磁共振波谱评估2-羟基戊二酸在胶质瘤异柠檬酸脱氢酶基因突变的研究意义

The significance of 2-hydroxyglutaric acid in glioma isocitrate dehydrogenase gene mutation assessed by magnetic resonance spectroscopy

摘要 目的:基于磁共振波谱检测2-羟基戊二酸(2HG)在异柠檬酸脱氢酶(IDH)基因突变状态的胶质瘤中的研究价值。方法:收集2021年1月—2022年6月在佛山市第一人民医院术后病理诊断为胶质瘤的患者21例,男性13例,女性8例,年龄27~73岁,中位年龄44岁,平均年龄46.2岁,病理分型:IDH基因突变组和IDH基因野生组,低级别胶质瘤组(Ⅰ~Ⅱ级),高级别胶质瘤组(Ⅲ~Ⅳ级)。所有患者扫描方案:MR平扫加增强及MRS。患者的IDH基因突变,采用Mann-Whitney U检验分析IDH基因突变组与IDH基因野生组间的2HG的差异,胶质瘤分级与IDH基因突变的相关性采用Spearman等级相关检验。结果:IDH基因野生组:14例,IDH基因突变组:7例,低级别胶质瘤组(Ⅰ~Ⅱ级):12例,高级别胶质瘤组(Ⅲ~Ⅳ级):9例。IDH基因突变组的2HG蓄积浓度为1.34(1.21,1.42)mmol/L,IDH基因野生组的2HG蓄积浓度为0.00(0.00,0.00)mmol/L。IDH基因野生组及突变组间2HG蓄积浓度差异有统计学意义(P<0.001),对鉴别诊断的灵敏度和特异度也最高,分别为85.7%、81.4%。IDH基因突变组2HG蓄积浓度与胶质瘤分级有一定的负相关性(rS=-0.545,P<0.001)。结论:2HG可预测IDH基因突变状态,对不同级别胶质瘤无创评估。 Objective Based on magnetic resonance spectrum detecting 2-hydroxy glutaric acid(2HG)in the state of isocitrate dehydrogenase(IDH)gene mutation of glioma research value.Methods From January 2021 to June 2022,21 patients with postoperative pathological diagnosis of glioma in Foshan First People's Hospital were collected.They were male:13,female:8,aged 27~73 years,median age 44 years,average age 46.2 years.Pathological types were as follows:IDH gene mutant group and IDH gene wild group,and low-grade glioma group(grade Ⅰ~Ⅱ),High-grade glioma group(Grade Ⅲ~Ⅳ).Scanning protocols for all patients:MR noenhanced scan plus enhanced scan and MRS.For the IDH gene mutation of patients,Mann‑Whitney U test was used to analyze the difference of 2HG between IDH gene mutation group and IDH gene wild group.Spearman rank correlation test was used to analyze the correlation between glioma grade and IDH gene mutation.Results IDH gene wild group:14 cases,IDH gene mutation group:7 cases.Low-grade glioma(grade Ⅰ~Ⅱ):12 cases,high-grade glioma(grade Ⅲ~Ⅳ):9 cases.The 2HG in the IDH gene mutation group was 1.34(1.21,1.42)mmol/L,and the 2HG concentration in the IDH gene wild group was 0.00(0.00,0.00)mmol/L.The difference of 2HG accumulation concentration between the wild group and the mutant group of IDH gene was statistically significant(P<0.001),and the sensitivity and specificity of IDH gene for differential diagnosis were also the highest(85.7% and 81.4%,respectively).There was a negative correlation between the concentration of 2HG and the grade of glioma in IDH mutant group(rS=-0.545,P<0.001).Conclusion 2HG can predict the status of IDH gene mutation in gliomas of different grades without invasive evaluation.
